Waterproofing spray-associated pneumonitis: review of 29 cases and comparisons with acute eosinophilic pneumonia and hypersensitivity pneumonitis

Abstract: Purpose: Waterproofing spray-associated pneumonitis (WAP) proceeds to acute respiratory failure and is characterized by diffuse bilateral ground-glass opacities on computed tomography. These features are similar to those of acute eosinophilic pneumonia (AEP) and hypersensitivity pneumonitis (HP). This study identified the characteristics of WAP and compared them with those of AEP and HP. Methods: We conducted a retrospective study of adult patients with WAP, AEP, and HP in Fukujuji Hospital from 1990 to 2018. … Show more

“…The inhalation of a waterproofing spray, especially a fluorinated compound resin used as a water repellent, causes ALI ( 2 ). In addition to the inhalation of a waterproofing spray, acute eosinophilic pneumonitis (AEP) and an exacerbation of chronic hypersensitivity pneumonitis (CHP) should also be considered in the differential diagnosis of early respiratory disorder following antigen inhalation ( 12 ). However, it is difficult to differentiate between these diseases based on the clinical findings.…”

“…The antigen inhalant associated with AEP and CHP is smoke and a repetitive specific antigen, respectively. Moreover, elevated eosinophils and lymphocytes in BALF are observed in patients with AEP and CHP, respectively ( 12 ). The BALF in ALI due to the inhalation of waterproofing spray are known to dominantly contain macrophages ( 12 ), but elevated neutrophils in the early phase after the inhalation of a waterproofing spray have also been reported ( 13 ).…”
