Waldenström-Like Immunocytic Lymphoma with IgG Serum M Component

Abstract: A case of immunoproliferative disorder with clinical features of Waldenström’s disease but with an IgG-k instead of IgM serum M component is described. The lymphocyte population in the bone marrow, blood and lymph nodes was studied by rosette test, culture with PHA and immunofluorescence staining. Most of the bone marrow lymphocytes had membrane IgG. This case represents an intermediate form between myeloma and Waldenström’s disease, thus supporting the Unitarian concept of all the immunoproliferative disorders Show more

“…In case 8 (previously re ported by Akasaka et al [1], the serum con tained 5 g/dl monoclonal IgG; however, clinical pictures resembled those of macroglobulinemia as in the cases reported by Ressegotti et al [15] and Tursz et al [17].…”

“…At least 30% of cases tested have an autoim mune haemolytic anaemia. A coagulation defect has been noted in this condition [2,9] although the exact mechanism is not en tirely clear. The cases reported show all of the features described previously.…”

“…Multiple myeloma accompanied with IgM paraproteinaemia and severe osteolytic lesions has been reported on several occasions (reviewed by Berman, 1975). More recently B-cell neoplasms have been described with a clinical history and morphological picture of Waldenstrom's disease but associated with an IgG or IgA production (Hijmans, 1975: Tursz er al, 1977Child et al, 1977;Resegotti et al, 1977). The presence of surface membrane and/or cytoplasmic isotypic determinants, identical with those of the serum M-component, has suggested a comparable monoclonal pleomorphic cell expansion as seen in Waldenstrom's disease.…”
