Her dört kadından üçünün yaşamları boyunca en az bir kez maruz kaldığı Candida vajiniti (CV)'nin en önemli risk faktörlerinden birisi de gebeliktir. Sunulan çalışmada, rutin gebelik takibi yapılan kadınlarda CV'nin prevalansı, trimesterlere göre dağılımı, etken mantar türlerinin belirlenmesi ve identifikasyon yöntemlerinin karşılaştırılması amaçlandı. Yöntem: Nisan 2021-Haziran 2021 tarihleri arasında hastanemize başvuran rastgele seçilmiş 250 gebeden posterior forniks sürüntü örneği alındı. Klinik örnekler, doğrudan mikroskop incelemesi, Gram boyaması, mantar kültürleri (CHROMagar Candida [CAC] ve Sabouraud glikoz agar) ile değerlendirildi. İzole edilen maya mantarları germ-tüp testi, mısır unlu-tween 80 agarda yapısal özelliklerinin değerlendirilmesi, CAC'da üreme özellikleri ve MALDI-TOF MS yöntemi ile identifiye edildi. Bulgular: Mantar vajinitlerinin prevalansı %43.2 idi. Mantar vajinitli 108 olgunun 22'si (%20.4) rekürren vajinit atakları tarif etti ve olguların %82.4'ü ikinci ve üçüncü trimesterde idi (p>.05). En sık izole edilen mantar türü Candida albicans (%49.6) olup, diğer türler C. glabrata (%35.3), C. krusei (%7.5), C. kefyr (%3.4), C. tropicalis (%1.7), Saccharomyces cerevisiae (%1.7) ve C. dubliniensis (%0.8) idi. Polifungal popülasyon 10 olguda (%9.3) görüldü ve en sık C. albicans + C. glabrata (%60) birlikteliği bulundu. Sonuç: Geçtiğimiz 10 yıl içerisinde, hastanemizde vajinit etkeni olarak belirlenen C. albicans oranı azalma eğiliminde iken, C. glabrata ve C. kruseideki artış endişe vericidir.

Background: Chronic vulvovaginal candidiasis is a major problem and common with poor quality of life from itching and discharge and it is difficult to treat because fungi are slow growers.Methods: Two hundred cases recruited from JAM clinic in Benha city, Egypt divided into two groups to study combination of oral itraconazole and local isoconazole in comparison with the use of local isoconazole only in chronic monilial vulvovaginitis and main outcome was relief of symptoms and signs with absent laboratory evidence of monilial infection by culture. Group one included 100 cases treated with combined oral itraconazole with three alternate days dose of local isoconazole (gynotrazonagen) and group two (local isoconazole only) included 100 cases treated with local isoconazole ovules on alternate days for 3 doses and week period.Results: Regarding the cure rate there were 90 clinically cured rate in the combined group compared to 70 in the local treatment group with p value of 0.0004 which was a very high statistically significant difference. Clinical cure included alleviation of the symptoms of discharge dysuria dyspareunia burning itching with also by clinical examination cure and disappearance of redness erythema edema fissures with absent pseudomycelia by microscopic examination. Usually, a symptom score of less than 2 is a positive one. Regarding the laboratory cure which means absent or negative candida culture on Sabaraud agar medium there were a laboratory cure rate of 84 out of 100 in the combined treatment group compared to 65 out of 100 in the local treatment group with p value of 0.002 and a high statistically significant difference.Conclusions: Oral itraconazole combined with local isoconazole is highly effective in treating chronic monilial vulvovaginitis.

PurposeAntifungal drugs are used frequently in the treatment of vulvovaginal candidiasis (VVC), but have shown controversial results. In this study, we aimed to evaluate the effectiveness of different antifungal drugs in the treatment of VVC and to provide an evidence-based reference for clinical use.MethodsThe published studies on the effectiveness of antifungal drugs in the treatment of VVC (up to April 2018) were retrieved from PubMed, Embase, the Cochrane Library, and Clini-calTrials.gov. We sifted through the literature according to Patients, Interventions, Comparisons and Outcomes principle, extracted data on the basic characteristics of the study, and evaluated the quality of included studies. We used R software for statistical analysis.ResultsIn total, 41 randomized controlled trials were included in this meta-analysis. The relative risk of VVC associated with ten drugs, including placebo, fluconazole, clotrimazole, miconazole, itraconazole, ketoconazole, econazole, butoconazole, terbinafine, and terconazole, was analyzed. The following drugs appeared to show more efficacy than placebo in the treated patients: fluconazole (OR =6.45, 95% CrI 4.42–9.41), clotrimazole (OR =2.99, 95% CrI 1.61–5.55), miconazole (OR =5.96, 95% CrI 3.17–11.2), itraconazole (OR =2.29, 95% CrI 1.21–4.33), ketoconazole (OR =2.40, 95% CrI 1.55–3.71), butoconazole (OR =1.18, 95% CrI 1.06–1.31), and terconazole (OR =5.60, 95% CrI 2.78–11.3). The value of surface under the cumulative ranking curve of each drug was as follows: placebo (0.5%), fluconazole (91.5%), clotrimazole (61.8%), miconazole (33.8%), itraconazole (50.5%), ketoconazole (42.8%), econazole (46.8%), butoconazole (82.2%), terbinafine (20.9%), and terconazole (65.0%).ConclusionAntifungal drugs are effective in the treatment of VVC. Fluconazole appeared to be the best drug for the treatment of VVC according to our analysis.
