Vulvodynia—new and more effective approaches to therapy

Abstract: Two cases are described of treatment-resistant vulvodynia that responded well to gabapentin. Gabapentin, an anti-epileptic drug, has been used in the treatment of neuropathic pain such as diabetic neuropathy and post-herpetic neuralgia. However, there has been little experience of its use in the relief of symptoms in vulvodynia and we add our observations to the one report of its use in these circumstances that has been published so far.

“…A total of nine articles were identified for possible inclusion in the review. The search yielded one open‐label trial ; six nonexperimental studies, i.e., one nonrandomized prospective cohort study of patients receiving sequentially applied treatment protocols including anticonvulsants , three retrospective case series , and two retrospective chart reviews ; and finally, two case reports . There were no randomized controlled trials identified.…”

“…26 This work has been borne out in other studies. 27 Glossodynia, a psychosomatic disorder with painful sensations in the mouth and throat, especially of the tongue, which have no definable cause, also responds to gabapentin. 28 Gabapentin is useful in treating "hot flashes" and hypothermia, and thus temperaturesensitive dermatoses.…”

“…In some cases the dynias occur secondarily, but more often, despite an exhaustive evaluation, no etiology is found. Two case series (8,9) of vulvodynia have shown good clinical response to gabapentin.…”
