Background and Aims: People who inject drugs (PWID) and other marginalized populations with high hepatitis C virus (HCV) infection rates represent a unique challenge for treatment initiation due to health, administrative and social barriers. We analysed the HCV cascade of care (CoC) in some vulnerable subpopulations in Madrid, Spain, when using a mobile point-of-care (PoC).Methods: From 2019 to 2021, a mobile unit was used to screen active HCV using a linkage-to-care and two-step PoC-based strategy. Viremic participants were grouped into four subgroups: PWID, homeless individuals and people with a mental health disorder (MHD) and alcohol use disorder (AUD). Logistic regression, and Cox and Aalen's additive models were used to analyse associated factors and differences between groups.Results: A prospectively recruited cohort of 214 HCV-infected individuals (73 PWID, 141 homeless, 57 with a MHD and 91 with AUD) participated in the study. The overall HCV CoC analysis found that 178 (83.1%) attended a hospital, 164 (76.6%) initiated direct-acting antiviral therapy and 141 (65.8%) completed therapy, of which 99 (95.2%) achieved sustained virological response (SVR). PWID were significantly less likely to initiate treatment, whereas individuals with AUD waited longer before starting the treatment. Both people with AUD and PWID were significantly less likely to complete HCV treatment.Conclusions: Overall, SVR was achieved in the majority of the participants treated.However, PWID need better linkage to care and treatment, whereas PWID and AUD need more support for treatment completion. It has been argued that human-centred security design needs to accommodate the considerations of three dimensions: (1) security, (2) usability and (3) accessibility. The latter has not yet received much attention. Now that governments and health services are increasingly requiring their citizens/patients to use online services, the need for accessible security and privacy has become far more pressing. The reality is that, for many, security measures are often exasperatingly inaccessible. Regardless of the outcome of the debate about the social acceptability of compelling people to access public services online, we still need to design accessibility into these systems, or risk excluding and marginalising swathes of the population who cannot use these systems in the same way as abled users. These users are particularly vulnerable to attack and online deception not only because security and privacy controls are inaccessible but also because they often struggle with depleted resources and capabilities together with less social, economic and political resilience. This conceptual paper contemplates the accessible dimension of human-centred security and its impact on the inclusivity of security technologies. We scope the range of vulnerabilities that can result from a lack of accessibility in security solutions and contemplate the nuances and complex challenges inherent in making security accessible. We conclude by suggesting a number of avenues for future work in this space.

Online users require a working knowledge of password “best practice”, as well as the ability to apply such knowledge. Children increasingly operate as independent agents online, and thus also need to be aware of password “best practice”. To meet this need, the Scottish curriculum for excellence includes lessons about password “best practice”. Hence, all Scottish children ought, theoretically, to have similar levels of password-related knowledge. They ought also, by age 8-9, to be able to apply their knowledge. One factor that could deter password-related knowledge acquisition and skill development is financial deprivation. To gauge its impact, we assessed the knowledge and abilities of Scottish 8-9 year old children, in four primary schools, in areas of varying financial deprivation. We uncovered stark differences in knowledge and password retention. There is a clear need for an extra-curricular intervention programme to teach up-to-date password “best practice” and support in developing the required password management skills. This will reduce their online vulnerabilities, whatever their socio-economic background.
