Voriconazole Use in Children: Therapeutic Drug Monitoring and Control of Inflammation as Key Points for Optimal Treatment

Abstract: Voriconazole plasma concentrations (PC) are highly variable, particularly in children. Dose recommendations in 2–12-year-old patients changed in 2012. Little data on therapeutic drug monitoring (TDM) after these new recommendations are available. We aimed to evaluate voriconazole monitoring in children with invasive fungal infection (IFI) after implementation of new dosages and its relationship with safety and effectiveness. “…This may be related to the following factors: (1) the hepatic blood flow in children is higher than that in adults; (2) the first‐pass effect of VRZ in children is more obvious; (3) the protein expression level of CYP2C19 in children's liver tissue is higher than that in adults 46,47 ; and (4) the activity of flavin monooxygenase 3 (FMO3) in liver microsomes in children aged 2–10 years is different from that in adults 48 . However, the results of another two studies in children were inconsistent with these findings; in one study, the CRP levels were not significantly related to VRZ concentrations, 25 while in the other, elevated CRP levels were significantly associated with insufficient VRZ concentrations 41 .
