Nanocrystalline hydroxyapatite (NHA) is a biocompatible, biodegradable, and osteoconductive bone graft material; however, it lacks osteoinductivity. The present study is aimed at investigating the feasibility of nanocrystalline hydroxyapatite (NHA) as an osteoinductive growth factor carrier. Bone morphogenic protein 2 (BMP2), an osteoinductive growth factor, was incorporated into NHA (BMP2-NHA) using a simple adsorption method. The growth factor loading and release kinetics were profiled using fluorescein-isothiocyanate-labeled bovine serum albumin (FITC-BSA) as a mimic of the osteoinductive growth factor BMP2. The effect of BMP2-NHA on the osteogenic differentiation of C2C12 cells and ectopic bone formation in mice were tested. Confocal laser-scanning microscopy showed that FITC-BSA was diffused throughout the porous structure of NHA. FITC-BSA was efficiently loaded in NHA and sustained release was observed up to 35 days in vitro. BMP2-NHA enhanced the expression of osteogenic markers Runx2, Osterix, Alp, and Col1α1 and ALP activity in C2C12 cells compared to NHA. Similarly, μ-CT and histological examinations showed that BMP2-NHA robustly induced ectopic bone formation in mice. This study suggests that NHA could be used as an effective carrier of osteoinductive growth factors, which ensures osteoinductivity of NHA via sustained release of the growth factor.

Improved handling characteristics of bone substitute materials may facilitate surgical handling. However, the respective modifications should not alter the biological activity. For comparison of a granular and a putty form of two silicaenhanced bone substitute materials, in 30 rabbits, bilateral bone defects were created in the femoral condyle and randomly filled with (1) sintered, silica-enhanced (0.8 wt%) hydroxyapatite (ACg, n ¼ 15); (2) ACg plus resorbable polymer (ACp, n ¼ 15); (3) non-sintered silica-enhanced hydroxyapatite (hydroxyapatite/SiO 2 ratio:61:39 wt%; NBg, n ¼ 15); and (4) NBg plus matrix of resorbable polymer (NBp, n ¼ 15). After 4, 8, and 12 weeks, respectively, newformed bone, remaining bone substitute materials, and amount of soft tissue were quantified histomorphometrically. After four weeks, new-formed bone was increased for NBg/NBp in which NBp showed significantly less remaining bone substitute materials when compared to ACg. After two months, the highest amount of new-formed bone and the least remaining bone substitute materials were seen for NBg and NBp. ACp had less remaining bone substitute material when compared to ACg. In addition, amount of soft tissue was significantly enhanced in NBp when compared to ACg and NBg. After 12 weeks, NBp had significantly lower remaining bone substitute material when compared to ACg. Amount of soft tissue was significantly increased in NBp when compared to ACg and NBg. In accordance, based on an early increase of new-formed bone, a lower amount of remaining bone substitute material, and as well as a distinct degradation, superiority of the non-sintered bone substitute material to the sintered bone substitute material was seen. The effect of the resorbable polymer to each bone substitute material can be supposed to be rather marginal.

Do Clinical and Radiological Assessments Contribute to the Understanding of Biomaterials? Results From a Prospective Randomized Sinus Augmentation Split-Mouth Trial

The present prospective randomized split-mouth trial reports on the 3-year clinical and radiological follow-up investigation of implants placed 7 months after sinus augmentation with 2 different bone substitute materials. The aim of the study was to complete the histologic observation of cellular reactions by analyses of the implants and the volumetric changes of the augmented bone substitute materials. A sinus augmentation split-mouth trial was performed in 14 patients with the synthetic bone substitute material Nanobone (NB) and the xenogeneic Bio-Oss (BO). Changes in volume and density of the augmented biomaterials were investigated by analysis of computed tomography scans, taken immediately after augmentation and after 7 months. Clinical implant parameters were assessed after 3 years of loading. Both bone substitute materials underwent nonsignificant volume reduction and significant increase in bone density over an integration period of 7 months. No significant differences concerning volume and bone density were observed between the groups. Three years after loading, 51 of 53 implants were in situ with no peri-implant infections, and only a few soft-tissue variations were present. The present prospective randomized study showed that no differences could be observed clinically and radiologically. Accordingly, it seems that both biomaterials, independent of their physicochemical composition, enable clinical success and long-time stability for dental implants. Interestingly, the histological results showed distinct differences in cellular reactions: While the xenogeneic BO induced a mild tissue reaction with only few multinucleated giant cells and comparably low vascularization, the synthetic NB induced a multinucleated giant cell-triggered tissue reaction with an increase of vascularization. Thus, the present study showed that a combination analysis-histological, clinical, and radiological-is necessary for a detailed assessment of a biomaterial's quality for clinical application.
