Introduction:We assessed the effect of different voiding positions on uroflowmetric parameters and post-void residual (PVR) urine volume in symptomatic benign prostatic hyperplasia (BPH) patients. We also evaluated the effect of alpha blockers on PVR in different voiding positions. Methods:The study was performed with 110 BPH patients over 50 years old. In total, 4 uroflowmetries were performed in all patients: 2 patients in the sitting position and 2 in the standing position. PVR was measured with transabdominal ultrasonography. Also, patients were divided into two groups according to the alpha adrenergic blocker treatment; the effect of this treatment on their uroflowmetric parameters in different positions was evaluated. Results: Maximum flow rate (Qmax) and average flow rate (Qave) were significantly higher in patients in the sitting position, but there were no differences in other uroflowmetric parameters and PVR volume (Qmax: 15.5±5.9 mL/s vs. 13.7±5.2 mL/s, Qave: 11.4±4.6 mL/s vs. 10.7± 3.9 mL/s, respectively; p < 0.05). The Qmax and Qave were significantly higher in sitting position, compared to the standing position, in both alpha adrenergic treatment and non-treated groups; again, there were no differences in other uroflowmetric parameters and PVR volume. Conclusion: Qmax and Qave values were significantly higher in the sitting position. Alpha blockers did not affect any change.

The reactions of some 1,3-dicarbonyl compounds with cycloheptatriene derivatives in the presence of Mn(OAc) 3 were examined. Cycloheptatriene forms mainly [2+3] and [6+3] dihydrofuran addition products derived from cycloheptatriene. However, the reaction of acetylacetone with cycloheptatriene substituted with an electron withdrawing group exclusively gave products derived from the norcaradiene structure. The formation mechanism of the products as well as the role of the substituent attached to cycloheptatriene is discussed.

ÖZETAmaç: Bu çalışmanın amacı, benign prostat hiperplazisi (BPH) olan hastalarda infravezikal obstrüksiyon derecesini tahmin etmekte kulanılan uluslararası prostat semptom skoru (IPSS) ile üroflovmetri, prostat volümü ve postmiksiyonel rezidüel idrar (PVR) miktarı arasındaki korelasyonu değerlendirmektir.Yöntemler: Üroloji polikliniğine Ocak 2010 ile Aralık 2010 tarihleri arasında başvuran işeme şikayeti ile muayene edilen 134 hasta retrospektif olarak incelendi. Hastalar IPSS değerlerine göre üç gruba ayrıldı; 1. Grup IPSS'i hafif olan hastalar (n=35 hasta), 2. Grup IPSS'i orta olan hastalar (n=67) ve 3. Grup IPSS'i olan hastalar (n=32). Hastaların IPSS ile prostat volümü, PVR ve üroflovmetri bulguları arasındaki ilişki karşılaştırıldı. Prostat hacimleri ile PVR suprapubik ultrasonografi ile değerlendirildi. İşemeyi etkileyebilecek üriner sistem rahatsızlıkları olan hastalar çalışmaya dahil edilmedi.Bulgular: Çalışmaya dahil edilen hastaların ortalama yaşı 55.6 (43-88) iken tüm grubun ortalama IPSS'si 13.5 (0-35) idi. Üçüncü gruptaki prostat hacimleri ve rezidüel idrar miktarları 1. Gruba ve 2. Gruba göre istatistiksel olarak anlamlı derecede daha fazla idi. Ortalama mesane duvar kalınlığı 2. Grup ve 3. Grup ile karşılaştırıldığında,1. The patients were distributed in three groups according to IPSS values; 1 st , lower IPSS group (n=35), 2 nd , moderate IPSS group (n=67) and 3 rd , higher IPSS group (n=32). IPSS, prostate volume, postvoiding residue (PVR), and maximum urine flow (Qmax) values were compared. Prostate volumes and PVR were evaluated with suprapubic ultrasound. The patients with comorbidities which may affect voiding function were excluded from the study. Results:The mean age and IPSS of study patients were 55.6 (43-88) and 13.5 (0-35) respectively. Mean prostate volume and PVR of the 3rd group were significantly higher compared to the 1 st and 2 nd groups. Conclusion:Although IPSS is a subjective finding of infravesical obstruction, a higher score in patients with lower urinary tract symptoms is in parallel with the objective parameters such as Qmax and PVR.
