Objectives/Hypothesis: Vocal process granuloma has been attributed to intubation, laryngopharyngeal reflux, and phonotraumatic/hyperfunctional vocal behaviors. Vocal process granuloma has recurrence rates following surgical excision approaching 92%. We hypothesize that a portion of persistent or idiopathic cases of vocal process granuloma result from underlying glottal insufficiency (GI) caused by paresis, scar, or atrophy. Our goal was to examine our vocal process granuloma population and determine the incidence of GI, treatment interventions, and outcomes.Study Design: Retrospective chart review. Methods: Thirty-four patients with vocal process granuloma were divided into surgically or conservatively managed groups. Patients were identified if they carried a diagnosis of GI. The time to resolution and number of recurrences within the overall treatment period was recorded and compared between subgroups. Pre-and post-treatment Voice Handicap Index-10 (VHI-10) and Reflux Symptom Index (RSI) scores were compared.Results: Eighteen of 34 patients (53%) carried an underlying diagnosis of GI, 13/34 (38%) were treated surgically, and 8/13 (62%) surgical patients had underlying GI. VHI-10 and RSI scores significantly improved after disease resolution (P < .05).Conclusions: The incidence of GI among patients with vocal process granuloma was 53%. Conservative therapies including treatment of laryngopharyngeal reflux and voice therapy may lead to resolution despite underlying glottal incompetence. If conservative measures fail, recognizing and treating glottal incompetence with true vocal fold augmentation may lead to a shorter surgical treatment course.

Гранулема гортани -опухолеподобное образование, ограничивающее подвижность голосовых связок, харак-теризуется неспецифическим воспалением c компактной инфильтрацией зрелых и активных мононуклеарных фа-гоцитов. Заболевание встречается довольно редко в ото-риноларингологической практике: 0,9-2,7% от всех рас-стройств голосовых связок у взрослых. Особенностью за-болевания является высокая частота рецидивов после хи-рургического лечения. Недостаточная изученность этио-логии болезни, ее рецидивирующий характер, отсутствие предикторов выздоровления или возможного рецидива, необходимость повторных оперативных вмешательств де-лают гранулему гортани актуальной проблемой, разработ-ка которой требует междисциплинарного подхода [1].К наиболее частым факторам, провоцирующим рост гранулемы, относятся гиперфункциональное использова- Цель исследования -психопатологический и клинико-психологический анализ на основе аффективно-стрессовой модели расстройств тревожно-депрессивного спектра (РТДС) при воспалительном оториноларингологическом заболевании -гранулеме гортани. Материал и методы. Были обследованы 30 пациентов в возрасте 33-65 лет. У 13 больных был рецидив гранулемы после оперативного вмешательства, у 17 -впервые выявленная гранулема. Результаты и заключение. Установлена связь гранулемы гортани с расстройствами тревожно-депрессивного спектра (РТДС). Последние имели преимущественно хроническое течение. Определена значимая роль длительно действующих психотравмирующих факторов в развитии РТДС. Подтверждена роль тревоги в структуре РТДС, а также значимость их диагностики для выбора терапевтической тактики. Психофармакотерапия привела к снижению частоты рецидивов гранулемы. Полученные данные подтверждают продуктивность использования аффективно-стрессовой модели заболевания при анализе РТДС с учетом типа аффективности. To perform a psychopathological and clinical/psychological analysis of anxiety-depressive spectrum disorders (ADSD) in inflammatory otorhinolaryngological disease -larynx granuloma. Material and methods. We examined 30 patients, aged 33-65 years, including 13 patients with post-surgical recurrence of granuloma and 17 patients diagnosed for the first time with granuloma. Results and conclusion. The relationship between larynx granuloma and ADSD, predominantly of chronic type was shown. Prolonged psychotraumatic factors play a significant role in the development of ADSD. A role of anxiety in the ADSD structure as well as an importance of their diagnosis for a choice of therapeutic tactics was confirmed. Psychopharmacotherapy has led to a reduction in the frequency of granuloma relapses. Гранулемы в зоне голосового отростка черпаловидного хряща (контактные) развиваются вследствие микротравмы, хро-нического кашля, гастроэзофагеального рефлюкса, перенапряжения голоса и психосоматических особенностей. В статье приведены данные обзора литературы по проблеме лечения гранулем гортани: хирургическое лечение показано при неэффективности консервативного лечения, а также при наличии образований, стенозирующих просвет гортани и требу-ющих морфологической верификации. В Клинике болезней уха, горла и носа Первого МГМУ им. И.М. Сеченова с 2014 по 2017 г. наблюдалось 8 пациентов: одна пациентка с постинтубационными гранулемами успешно прооперирована, реци-дива нет в течение 6 месяцев. Семь пациентов проходят лечение по поводу гастроэзофагеальной рефлюксной болезни, на фоне чего в одном случае отмечен полный регресс гранулемы; пяти больным проведено хирургическое удаление грану-лемы, из них у трех не было рецидива, а у двух отмечено повторное появление гранулем спустя 6 месяцев, по поводу чего они реоперированы. Интраоперационное применение радиоволнового аппарата при удалении гранулемы с последующей коагуляцией места ее прикрепления очевидно уменьшает риск рецидива. bothsides granuloma underwent a surgical excision under general anesthesia, followed up for 6 months and had no recurrence. 7 patients (6 males and 1 female) were treated with proton pump inhibitors due to they had gastroesophageal reflux disease. In 1 case contact granuloma reduced after 3 month conservative therapy without recurrence. One patient was non compliant. In 5 others microsurgical treatment performed. 2 of them had recurrent granuloma after 6 months and underwent repeated microsurgical excision using radiofrequency ablation. According the world and our own experience high-energy surgical equipment is recommended to be used in recurrent laryngeal granuloma to reduce it's recurrence rate.
