Purpose: To describe and evaluate demographics, clinical features, prognostic factors, rate of success of surgery, incidence and visual outcomes in patients with a late recurrence of rhegmatogenous retinal detachment(RRD) over a 10-year period at a large tertiary referral eye centre. Methods: A retrospective, observational case-series of patients with late recurrence of retinal detachment, defined as redetachment after at least 6 months of total reattachment in non-PVR RRD, after vitrectomy(PPV) surgery with gas tamponade. Results: 39 patients with a late recurrence of RRD out of 16,396 RRD operations. The mean of time between the first RD surgery and redetachment was 122.7 weeks(SD 115). On presentation with late recurrence, 72% of eyes were pseudophakic and 64% were macula-off. In 28 eyes small breaks were found. 38% had established PVR,(PVR-C in 80 %). 95% underwent PPV. Gas was used in 61%.The initial secondary success rate was 41%. Initial BCVA was 1.32 logMAR(6/120) and final was 0.8 logMAR(6/38)(p-value 0.002). Conclusion: Late recurrence of retinal detachment is rare. It is characterized by small retinal breaks which may be difficult to visualise. Although cases can be treated with favourable anatomical results, visual outcomes are often less good, the success rate is lower.

Purpose: To describe a new technique of suturing sclerotomy after 23- gauge pars plana vitrectomy using10 – 0 nylon suture with releasable knot. Study Design: Quasi experimental study. Place and Duration of Study: Sahiwal Teaching Hospital, Sahiwal, from January 2022 to June 2022. Methods: 10 – 0 nylon sutures with releasable knots were applied to close the sclerotomy after 23 – gauge pars plana vitrectomy in 100 cases. Complications like hypotony and wound leakage were recorded. Intra ocular pressures on 1st post-operative day and after 1 week were monitored. Inflammation at suture site, pain experienced by the patient and surgeon’s convenience of suture removal under a slit lamp after 1 week were documented. Results: Patients included 58 women and 42 men. Post-operatively, there was no wound leakage and hypotony in any patient, with mean intra ocular pressure of 13.5 ± 2.0 mmHg on 1st post-operative day and 17.6 ± .0.5 mmHg after 1 week of surgery. In 4% cases inflammation was observed at suture site. The mean pain score, measured according to visual analogue scale, at post-operative day 1 and day 7 was 1.25 ± 1.19 and 1.08 ± 0.99 respectively. No discomfort was reported during removal of releasable sutures in 96% of the patients. Surgeon found it convenient to remove the sutures under slit lamp illumination. Conclusion: Closure of 23 – gauge sclerotomy using nylon 10 – 0 suture with releasable knot is a useful option. It prevents post-operative wound leakage and hypotony.

Frequency of Post-Operative Hypotony with Sutureless Sclerotomy versus Intrascleral Hydration in 23-Gauge Pars-Plana Vitrectomy

Purpose: To compare the frequency of hypotony after 23-gauge (G) pars plana vitrectomy (PPV) with suture-less sclerotomy versus intrascleral hydration for sclerotomy closure in eyes with air/gas tamponade. Study Design: Quasi-experimental study. Place and Duration of Study: College of Ophthalmology and Allied Vision Sciences, Mayo Hospital, Lahore from November 2021 to May 2022. Methods: A total of 90 eyes were included who underwent PPV with air/gas tamponade. One group underwent PPV with suture-less sclerotomy while the other got intrascleral hydration for sclerotomy closure. Intraocular pressure (IOP) was measured pre and post-operatively at day one and day seven by applanation tonometry in all eyes. IOP of 6 mmHg or less was defined as hypotony. Primary endpoint measure was rate of early post-operative hypotony. Results: A total of 90eyes of 90 patients, 40 (44.4%) males, and 50 (55.6%) females were included in the study. Five (5.6%) patients reported hypotony, out of which, only one (2.2%) case was seen in the intrascleral hydration group while rest (8.9%) were of the sutureless group. Sclerotomies requiring sutures were 4.44% (6 of 135) in sutureless group while 0.74% (1 of 135) in intrascleral hydration group. Paired sample T-test for the means of pre-operative and post-operative IOP was significantly different in both groups. For suture-less PPV, mean difference was 3.089 ± 7.960 mmHg (P = 0.013), while for the intra scleral hydration group, it was 3.778 ± 7.048 mmHg (p = 0.001). Conclusion: Intrascleral hydration is a suitable option for PPV sclerotomy closure without having any side effects.
