Vitrectomy in two cases of bilateral Terson syndrome

Abstract: We report on two patients with bilateral vitreous haemorrhage after a subarachnoid haemorrhage (Terson syndrome). We performed vitrectomy in four eyes with complete recovery of vision in three eyes; one eye had visual acuity of 1/6 due to preretinal fibrosis. The patient who had 20/20 vision in both eyes had a retinal detachment in one eye two months after the vitrectomy; this was operated upon and vision did not deteriorate.

“…Since then, many reports have been published focusing on surgical outcomes for predominately aneurysmal causes of Terson syndrome. 6,12,18,19,23 Although these reports have been generally applied to all causes of Terson syndrome, little has been published on outcomes after surgical management of vitreous hemorrhage induced by traumatic brain injury. To our knowledge, this report comprises the largest review to date of visual outcomes after vitrectomy for vitreous hemorrhage secondary to traumatic brain injury.…”

“…Several reports have demonstrated good visual recovery after vitrectomy surgery. [111][112][113][115][116][117] Schultz and associates 112 reported the longterm visual outcome of 30 eyes that were either observed or treated with pars plana vitrectomy in a retrospective, nonrandomized study with an average follow-up of 48 months. They found no significant difference in the average time to best vision for the vitrectomy versus observation groups but suggested that this may represent a selection bias.…”

“…Patients with subarachnoid haemorrhage from intracerebral aneurysms may develop intraocular bleeds (Terson 1900 ;van Rens et al 1983 ;Weingeist et al 1986 ;Schultz et al 1991 ;Kuhn et al 1998 ;Murjaneh et al 2006 ;Garweg and Koerner 2009 ) . Histopathology has shown haemorrhage in the vitreous, subhyloid, sub-ILM, intraretinal and in the optic nerve and the optic nerve sheath.…”
