Background: Childhood vitiligo is a special subtype and is seen in significant proportion of vitiligo patients. There are only a few clinical studies in the past which address the clinical spectrum of vitiligo in children. This study on eighty cases of childhood vitiligo will cover the epidemiology and clinical spectrum.Methods: To study the epidemiology, clinical spectrum and associations in childhood vitiligo. Inclusion criteria: all new cases of vitiligo in children under 12 years attending the outpatient department of Dermatology, exclusion criteria: old treated cases of vitiligo and age more than 12 years. Statistical analysis was done using mean and percentage of means.Results: Most common age group affected includes 4-6 years. Most common site of initial lesion was head and neck followed by upper limb, lower limb and trunk. Most common clinical type was vitiligo vulgaris followed by focal type then segmental. Lip tip type was least common type. Conclusions: Childhood vitiligo is a serious issue and the knowledge of its various patterns and associations needs to be updated at regular intervals.

Vitiligo is a disease characterized by discoloration of the skin and hair as a result of damage of melanocytes. The exact mechanism of the disease is still unknown. Theories about the pathogenesis focus on autoimmune, neurogenic and autocytotoxic mechanisms. There is a decreased risk of incidence of skin cancers in patients with vitiligo. The relation between vitiligo and decreased frequency of cases of malignant melanoma (MM) may be a consequence of the immune response to antigens, which are common for melanocytes and MM cells. Genetic studies indicate a mutually exclusive relationship of melanoma and vitiligo. This disease is also associated with a polymorphism in the TYR gene which encodes tyrosinase. Studies also indicate a decreased risk of incidence of nonmelanoma skin cancers (NMSC) in patients with vitiligo. streszczenie Bielactwo jest chorobą charakteryzującą się odbarwieniami skóry i wło-sów w wyniku uszkodzenia melanocytów. Dokładny mechanizm tej choroby jest nadal nieznany. Teorie na temat patogenezy koncentrują się wokół mechanizmów: autoimmunologicznych, neurogennych i auto cytotoksycznych. U pacjentów z bielactwem odnotowano zmniejszone ryzyko zachorowań na nowotwory skóry. Związek między vitiligo a zmniejszoną częstością występowania przypadków melanoma malignum (MM) może wynikać z odpowiedzi immunologicznej przeciwko antygenom, które są wspólne dla melanocytów i komórek czerniakowych. Badania genetyczne wskazują na wzajemne wykluczanie się bielactwa i czerniaka. Choroba ta również wiąże się z polimorfizmem w genie kodującym tyrozynazę (TYR). Badania wskazują także na zmniejszone ryzyko zachorowań na nieczerniakowe nowotwory skóry u pacjentów z bielactwem.

Objective: The present study aims to investigate the effects of vitiligo on quality of life (QoL), self-esteem (SE), and body image (BI). Patients and Methods:This study included 64 patients diagnosed with vitiligo, and 87 ageand sex-matched control subjects. Sociodemographic characteristics of the participants were collected and hypopigmented lesion involvement regions were recorded to identify clinical severity of vitiligo. The Dermatology Quality of Life Index (DQoLI), Body Image Scale (BIS), and Rosenberg Self-Esteem Scale (RSES) were completed. Results: Comparison of DQoLI, SE, and BI between vitiligo and control groups demonstrated that vitiligo patients had a statistically significantly lower QoL (P<0.01), a lower SE (P<0.01), and a worse BI (P<0.01). Localization of lesions at one or multiple sites did not show a statistically significant impact on DQoLI, SE, or BI (P>0.05). The DQoLI, BIS, and RSES scores and the levels of QoL, SE, and BI were similar between the patients with vitiligo for ≤5 and >5 years (P>0.05). Conclusion: Vitiligo can negatively affect the QoL, SE, and BI of patients, thereby, resulting in psychosocial problems. It can be concluded that the unfavorable skin appearance of the disease is not only a factor affecting the patients' QoL, SE, and BI; however, its relation with neurobiological mechanisms should be further investigated. This study emphasizes that body image and self-esteem is affected independently of the location of vitiligo lesions, and consequently neurobiological mechanisms may be important.
