Low blood levels of the vitamin D metabolite 25-hydroxyvitamin D [25(OH)D] have been associated with an increased risk and poorer outcomes of various cancers, including hematological malignancies. The Central Kazakhstan area has a relatively high incidence rate of leukemia. However, the relationship between vitamin D status and leukemia or other types of cancer in Kazakhstan has not yet been addressed. Therefore, in this first pilot single-center study conducted in Central Kazakhstan, we compared plasma levels of 25(OH)D and the vitamin D receptor (VDR) gene expression levels in peripheral blood mononuclear cells of patients with leukemia and demographically matching healthy volunteers. The levels of 25(OH)D in patients were found to be significantly lower (10.8 ± 7.0 ng/mL; n = 31) than in healthy subjects (21.6 ± 7.8 ng/mL; n = 34; p < 0.0001). A similar difference was observed in both younger (<60 years old) and older (>60 years old) participants, though there was no association between 25(OH)D concentration and age within the patient group. In female patients, 25(OH)D levels were significantly lower than in male patients (p = 0.04). No significant seasonal variations of 25(OH)D were observed in either the patient or the control group. VDR gene expression levels appeared to be similar in leukemia patients and healthy subjects, and no correlation between the cellular VDR expression and plasma 25(OH)D concentrations was observed in either group of participants. We did not observe a significant association of 25(OH)D or VDR levels and overall survival of leukemia patients. This observational study conducted for the first time in Kazakhstan supports previous findings demonstrating reduced blood 25(OH)D levels in cancer (leukemia) patients. Larger studies are required to determine whether low 25(OH)D plasma concentrations represent a risk factor for leukemia development and/or progression.

BACKGROUND: Over the past decades, patient satisfaction has been increasingly recognized as a powerful tool to measure the quality of health-care services. AIM: This study evaluated the satisfaction of systemic lupus erythematosus (SLE) patients with current clinical management in Semey, East Kazakhstan, and explored the factors associated with dissatisfaction. METHODS: All adult SLE patients registered at health-care facilities of Semey were enrolled in a cross-sectional study. RESULTS: Out of 67 patients, 66 were females and their mean age was 39 years. Symptoms associated with the musculoskeletal system were the most commonly reported (95.5%), followed by fatigue (88.1%) and neurological symptoms (53.7%). None of the patients were very satisfied or satisfied with current clinical management and the overall rate of patient dissatisfaction was 83.6%. Education (p = 0.04), monthly income (p = 0.01), SLE disease activity index score (p = 0.031), therapy with biologic agents (p = 0.029), immunosuppressants (p = 0.01), and corticosteroids (p = 0.01) were predictors of patient dissatisfaction in multiple logistic regression analysis. CONCLUSION: This real-world study is the first effort to understand the experience of SLE patients within the health-care system of Kazakhstan and the results may facilitate doctor-patient discussions on the initiatives that need to be taken to improve the quality of the medical services provided.

Introduction. Without magnesium, vitamin D cannot be converted to its biolo- gically active form, a relationship that is often overlooked. Objective. The aim of this study was to estimate the frequency of hypovitaminosis D and that of hypomagnese-mia on the one hand, and to analyze the relationship between vitamin D status, magnesium, and plasma parathormone on the other hand. Population and methods. A retrospective descriptive-analytical,and epidemiological study was conducted on a 112 apparently healthy young adult subjects. For this purpose, bioassays of 25-hydroxy-vitamin D, magnesium and plasma parathormone were performed. Results. The frequency of hypovitaminosis D (serum 25-hydroxyvitamin D  30ng/mL) represented 88.39%. Hypomagnesemia affeted 43.75% of the studied population. A highly significant positive correlation was noted between plasma magnesium levels and vitamin D status of the subjects (R= 0.849; P = 0.000), whereas, a very significant negative correlation was observed between magnesium and plasma parathormone levels (R=-0.92 ; P = 0.000). Conclusion. A high frequency of hypovitamin D, a significant frequency of hypomagne-semia, and a positive correlation between plasma magnesium and vitamin D status are observed. Magnesium level is a determining factor in the variation of serum calcidiol concentrations that should be considered in the assessment of vitamin D status.
