Vitamin D as an adjunctive therapy in asthma. Part 1: A review of potential mechanisms

Abstract: Vitamin D deficiency (VDD) is highly prevalent worldwide. The classical role for vitamin D is to regulate calcium absorption form the gastrointestinal tract and influence bone health. Recently vitamin D receptors and vitamin D metabolic enzymes have been discovered in numerous sites systemically supporting diverse extra-skeletal roles of vitamin D, for example in asthmatic disease. Further, VDD and asthma share several common risk factors including high latitude, winter season, industrialization, poor diet, ob… Show more

“…However, the effect of vitamin D on immune and inflammatory signals may outweigh any potential shifting of Th1eTh2 balance and the majority of existing mechanistic [92] as well as observational data suggest that vitamin D repletion may be therapeutic in asthma. In fact, some authors have suggested that VDD may be responsible for the increase in allergic diseases and asthma prevalence worldwide [171,107,106].…”

“…We will discuss the merits and limitations of each study design regarding asthma and vitamin D. However, unanswered questions remain. We have reviewed mechanisms by which vitamin D may influence asthmatic disease in part one of this two-part review [92]. Therefore, we will conclude this review with some recommendations for future research in the area.…”

“…Although current asthma therapies control the symptoms of the disorder, there is little evidence to suggest that the current therapies affect airway remodeling [61]. Given that lower levels of serum vitamin D has been shown to be associated with increased ASM mass and AHR, worse control of asthma and increased exacerbations in children, adolescents and adults [18,2022], vitamin D could be a potent modulator of ASM cells (Figure 2).…”

“…10 Low levels of vitamin D have been associated with asthma severity and loss of control, together with recurrent exacerbations. Remodeling is an early event in asthma described as a consequence of production of mediators and growth factors by inflammatory and resident bronchial cells.…”
