Vitamin B-12 deficiency after gastric surgery for obesity

Abstract: Low serum vitamin B-12 concentrations after gastric bypass (GB) surgery for obesity were observed in 11 of 28 patients without detectable impairment of crystalline vitamin B-12 absorption. This was observed in 2 of 19 patients with vertical banded gastroplasty (VBG). In contrast, protein-bound vitamin B-12 absorption was markedly impaired, as demonstrated in eight of these patients after GB (n = 7) and VBG (n = 1). Correction of this impaired absorption occurred when protein-bound vitamin B-12 was incubated wi… Show more

“…The majority of studies report deficiencies of ϳ35% (114,119 -121). Supplementation with crystalline B 12 has been found to prevent deficiency (119). A daily multivitamin is known to prevent folic acid deficiency (122), a problem that was reported in up to 38% of GBP patients (114,116,122,123) prior to food fortification with folate.…”

“…Vitamin B 12 de®ciency has been reported after gastric bypass surgery, but is uncommon after gastric reduction surgery. 17,18 This study focused on measurement of these parameters, party because of the dif®culty of assessing intake. These are in fact two of a limited range of biochemical markers of nutritional status 19 which are easy to measure.…”

“…Despite these preventive measures, 7% of patients undergoing this surgical treatment develop bone demineralization, usually between the second and fifth postoperative years 19 . Since vitamin B12, in its protein-bound form, is poorly absorbed in these patients, Rhode 30 has recently suggested daily administration of the crystalline form of vitamin B12 at very high dosage. Indeed, at least 350 ug of crystalline vitamin B12 are necessary to normalize the serum levels of this vitamin, and a dosage as high as 500 ug/day has been recommended 16 .…”
