Vital exhaustion, extent of atherosclerosis, and the clinical course after successful percutaneous transluminal coronary angioplasty

Abstract: It has been observed that vital exhaustion, a state characterized by unusual tiredness, increased irritability and feelings of demoralization not uncommonly precedes myocardial infarction in apparently healthy individuals. This observation raised the question as to whether vital exhaustion is a marker of subclinical coronary disease. To answer that question the condition was assessed in 105 male patients (mean age 54.8 year) before and 2 weeks after successful percutaneous transluminal coronary angioplasty (PT… Show more

“…Although vital exhaustion is considered a short-term risk factor and predicts cardiac mortality up to the 40th month post-myocardial infarction (MI); it is a risk factor, which is on a par with hyperlipidemia [13 -15]. One study has shown that vital exhaustion may decrease following medical intervention, but most patients still remained exhausted after percutaneous transluminar coronary angioplasty (PTCA) [16]. “…12,13 Evidence from studies examining the physiological correlates of exhaustion indicates that exhaustion, as defined in the current study, cannot be thought of as a somatoform or similar disorder, as listed in the Diagnostic and Statistical Manual of Mental Disorders (DSM IV-TR). Exhaustion has been related to increased inflammation, 14,15 impaired fibrinolysis, 16 and low vagal tone, 17 all of which have been associated with the pathogenesis of CAD.…”

“…Various studies have associated VE with CHD and chronic heart failure (Appels, Kop et al 1995;Pedersen and Middel 2001;Smith, Gidron et al 2009). VE has been associated with a twofold to threefold increased risk of mortality and morbidity in patients with CHD (Kop, Appels et al 1994;Appels, Kop et al 1995).…”

“…VE has been associated with a twofold to threefold increased risk of mortality and morbidity in patients with CHD (Kop, Appels et al 1994;Appels, Kop et al 1995). It has also been associated with sudden cardiac arrest and adverse cardiac events in patients that underwent successful angioplasty (Kop, Appels et al 1994;Appels, Golombeck et al 2000).…”
