Visual outcomes and complications of posterior chamber intraocular lens implantation in the first year of life

Abstract: Visual axis reopacification was the main complication of IOL implantation in infants, with PHPV leading to more complications and repeat procedures. Anterior vitrectomy appeared to reduce the reoperation rate. Results indicate that primary posterior capsulorhexis is important and Nd:YAG capsulotomy is not satisfactory in infants. In addition, the reduction in glaucoma with IOL implantation, if borne out over the long term, is a significant advantage in cases of congenital cataract.

“…Children with traumatic or complicated cataracts and those who underwent cataract extraction without PPC or AV were excluded. As the risk of developing postoperative complications is commoner in younger children, [5][6][7][8][9] we divided all the children into less and more than 2 years. As implantation of IOL can further influence intraoperative difficulties and postoperative complications like VAO, the children less than 2 years were further divided on the basis of IOL implantation.…”

“…Notably, most myopic shift occurs in the first 3 years of life. 6 Therefore, a child who was initially a poor CTL candidate or CTL-intolerant, spectacle intolerant, or suffering from anisometropic amblyopia not successfully managed with penalization will likely still be so when myopic shift occurs. IOL exchange should be considered an option to mitigate adverse refractive outcomes.…”

“…Em um estudo onde foram implantadas lentes intra-oculares em crianças menores que 1 ano de idade, os autores observaram uma média de 6 dioptrias de miopização pós-operatória nos 41 meses de seguimento (31) . Através de um seguimento pós-operatório de 3 anos de 52 olhos operados, os autores observaram uma miopização de 5,96 dioptrias em crianças operadas com idade inferior a 2 anos.…”
