Objective: This paper explores the therapeutic relationship, engagement and communication for participants using telepsychiatry (i.e., video; TP), telebehavioral health (TBH) and other technologies. Methods: The goal of this concept paper is to help clinicians, teachers and researchers consider the intersection of the following three topics: 1) the therapeutic relationship in psychiatric and behavioral health care; 2) how TBH affects the therapeutic relationship, engagement, communication, and boundaries; and 3) research findings on telepresence (TPr), virtual reality (VR) and augmented reality (AR) applied to clinical care. A keyword search focused on concept areas: 1) therapeutic relationship, engagement, communication, boundaries, clinical; 2) telepsychiatry, telemental or telebehavioral health, telepsychology); 3) psychotherapy via technology; 4) computer/technology-mediated communication; and 5) telepresence related to technologies (virtual, augmented, reality, environment). Results: From a total of 4,712 potential references, 453 were eligible for full text review and 77 papers directly related to the concepts. From papers' references and a review of books and popular literature, 12 other sources of information (i.e., papers, books, other) about TPr and VR were found. Though few studies literally discuss the therapeutic relationship, the impact of technology on clinical engagement and communication is growing, from clinical (e.g., psychotherapy) and non-clinical (e.g., communication, TPr) literature. The historical evolution of TPr and VR shows that definitions, foci of studies (e.g., social neuroscience to business) and assessments vary widely. TP and TBH care can be improved by concepts of TPr, avoiding distractions, understanding the technology-human interface, measures of TPR for clinical use and the use of simulation for training. Conclusions: Additional evaluation/research is needed to ensure quality care, training, and evaluation of the therapeutic relationship while preventing potential pitfalls.

Künstliche Intelligenz (KI) hat in den letzten Jahren eine neue Reifephase erreicht und entwickelt sich zum Treiber der Digitalisierung in allen Lebensbereichen. Die KI ist eine Querschnittstechnologie, die für alle Bereiche der Medizin mit Bild‑, Text- und Biodaten von großer Bedeutung ist. Es gibt keinen medizinischen Bereich, der nicht von KI beeinflusst werden wird. Dabei spielt die klinische Entscheidungsunterstützung eine wichtige Rolle. KI-Methoden etablieren sich gerade beim medizinischen Workflow-Management und bei der Vorhersage des Behandlungserfolgs bzw. des Behandlungsergebnisses. KI-Systeme können bereits in Bilddiagnose und im Patientenmanagement unterstützen, aber keine kritischen Entscheidungen vorschlagen. Die jeweiligen Präventions- oder Therapiemaßnahmen können mit KI-Unterstützung sinnvoller bewertet werden, allerdings ist die Abdeckung der Krankheiten noch viel zu gering, um robuste Systeme für den klinischen Alltag zu erstellen. Der flächendeckende Einsatz setzt Fortbildungsmaßnahmen für Ärzte voraus, um die Entscheidung treffen zu können, wann auf automatische Entscheidungsunterstützung vertraut werden kann.

We present VICA, a Visual Counseling Agent designed to create an engaging multimedia face-to-face interaction. VICA 1 is a human-friendly agent equipped with high-performance voice conversation designed to help psychologically stressed users, to offload 2 their emotional burden. Such users specifically include non-computer-savvy elderly persons or clients. Our agent builds replies 3 exploiting interlocutor's utterances expressing such as wishes, obstacles, emotions, etc. Statements asking for confirmation, details, 4 emotional summary, or relations among such expressions are added to the utterances. We claim that VICA is suitable for positive 5 counseling scenarios where multimedia specifically high-performance voice communication is instrumental for even the old or digital 6 divided users to continue dialogue towards their self-awareness. To prove this claim, VICA's effect is evaluated with respect to a 7 previous text-based counseling agent CRECA and ELIZA including its successors. An experiment involving 14 subjects shows VICA 8 effects as follows: i) the dialogue continuation (CPS: Conversation-turns Per Session) of VICA for the older half (age >40) substantially 9 improved 53% to CRECA and 71% to ELIZA. ii) VICA's capability to foster peace of mind and other positive feelings was assessed 10 with a very high score of 5 or 6 mostly, out of 7 stages of the Likert scale, again by the older. Compared on average, such capability of 11 VICA for the older is 5.14 while CRECA (all subjects are young students, age<25) is 4.50, ELIZA is 3.50, and the best of ELIZA's 12 successors for the older (>25) is 4.41.13
