The new computer-based interactive technologies in medicine, such as virtual reality (VR), have revolutionized education. The use of virtual microscopic images would be invaluable in the training of cyto-histopathologists. However, due to the vast amount of digital information on a scanned, conventional cyto-histological slide, which is enormous by current data storage standards, these systems are expensive and not widely used in pathological medicine. The authors propose an inexpensive system based on quicktime virtual reality (QTVR) technology (by Apple Computers Inc.), which accommodates a wide area of a slide at high magnification, generating a 'virtual slide' which makes it possible to navigate by conventional input devices. Commercial softwares that stitch consecutive, adjacent images of cyto-histological preparations onto a QTVR panorama were used. QTVR files have the ability to stand on their own as self-contained, multimedia applications and also have the ability to generate multinode scenes by means of 'hot spots'. QTVR 'movies' can be played on Macintosh or Windows platforms, and on major web browsers. Virtual slides by QTVR is an inexpensive system of high educational value, which allows the creation of multimedia databases of cyto-histological preparations that can exist on an internet server or can be distributed on removable media.

Background/Aim: Peritonitis rates in peritoneal dialysis (PD) vary considerably not only across countries but also between centers in the same country. Patient education has been shown to significantly reduce infection rates but up till now training lacks standardization with patients being trained using different methods and media (e.g., illustrations, videos). As a result, patients may be insufficiently experienced in performing PD, which might be one of the causes for high peritonitis rates. To address these issues, we developed a PD training program based on virtual reality (VR). Methods: To become acquainted with the PD procedure, patients are equipped with a VR headset and controllers. They are presented with a virtual PD set, which simulates the feeling of sitting in front of a real PD set. The patient is enabled to run through the program as often as necessary to become familiarized with the whole PD procedure. The aim is to standardize, facilitate, and accelerate the individual learning process. To compare the effect of the applied training method to traditional training, a randomized controlled trial is underway. Conclusion: Previous studies on the effectiveness of learning showed that VR training applications are superior to traditional methods, such as text- or video-based training. However, no study has been undertaken in the context of dialysis. We believe that the implementation of VR training programs in clinical practice will be beneficial in improving the patient’s proficiency, and thereby the quality and safety of PD.

Grundlagen: Die Simulation durch virtuelle Realität gewinnt in der chirurgischen Ausbildung zunehmend an Bedeutung.Methodik: Diese Übersicht stellt Simulatoren für die chirurgische Ausbildung vor.Ergebnisse: Derzeit stehen verschiedene Simulatoren zur Verfügung, mit denen laparoskopische und endoskopische Techniken simuliert und trainiert werden können. Die Datenlage zeigt, dass diese Simulatoren eine wichtige Rolle in der chirurgischen Ausbildung und zum Erlernen des chirurgischen Handwerks spielen.Schlussfolgerungen: Bei adäquater Anwendung ist die virtuelle Realität anderen Trainingssystemen überle-gen. Durch Objektivierung, Reproduzierbarkeit und Messbarkeit der handwerklichen Vorgänge kann eine vorteilhafte Ergonomie erlernt werden. Zunehmende Erfahrung mit zahlreichen Simulatorsystemen zeigt, dass diese prinzipiell sehr vorteilhaft für Lehre und Ausbildung in der Chirurgie sind. Background: Virtual-reality simulation in surgical training has become more widely utilized and intensely investigated as the result of changes in residency structure and the need to develop safer, more efficient, and measurable training processes.Methods: The use of surgical virtual-reality devices and their evolution from simple nonanatomic and nonhaptic basic-skills trainers to whole-procedure and part-task procedural simulators are reviewed.Results: A range of virtual-reality devices can currently be used to train laparoscopic manipulative tasks and flexible endoscopic tasks. Current data indicate that these training devices are of significant value in building surgical resident skills. Newer devices can be enabled with haptics, mentoring, and enhanced performance measurement capabilities.Conclusions: If executed properly, virtual reality offers inherent advantages over other training systems. The most important of these is facilitation of performance measurement, which makes proficiency-based training feasible. Accruing experience with multiple simulation systems indicates that this type of training is valuable and accessible. Significant challenges remaining to be overcome before wider implementation of training and assessment with these devices can occur include validation as well as the identification of resources for procurement and ongoing development.
