Injury-related hospitalisations in a rural county in China from 1994 to 2005 were analysed for trend and rate by demographics. Traffic-related hospitalisation increased from 25.5 per 100 000 in 1994-1996 to 57.9 in 2003-2005, and overtook assaults as the leading cause of injury. Motorcyclists, pedestrians and bicyclists accounted for 41%, 22% and 19% of traffic-related hospitalisations. Compared with females, males had a higher risk of traffic-related hospitalisations (rate ratio (RR) 2.38, 95% CI 1.89 to 3.00), falls (RR 1.64, 95% CI 1.11 to 2.42) and assaults (RR 4.29, 95% CI 3.23 to 5.69). Relative to 25-59-year-olds, 15-24-year-olds were at increased risk of traffic crashes (RR 1.76, 95% CI 1.37 to 2.25) and assaults (RR 1.56, 95% CI 1.21 to 2.01), and adults aged 60 years or older were at increased risk of falls (RR 2.58, 95% CI 1.61 to 4.14). Priority should be given to prevention of traffic injuries among motorcyclists, pedestrians and bicyclists, assaults among male adolescents and young adults, and falls among older adults.

AMAÇBu yazıda, İran'ın kentsel nüfusundaki kafatası kırık tipi oluş mekanizması tanımlandı. GEREÇ VE YÖNTEMGenel özellikler, travma mekanizması, kısaltılmış travma skalası, Glasgow koma skalası, hastanede kalış süresi ve 1999 yılından 2004 yılına kadar kaydedilen travma hastalarının sonucunu içeren verilere, İran Ulusal Travma Kayıt-ları veritabanından ulaşıldı. BULGULARKayıtlı 16,321 travma hastası arasında, kafatası kırığı olan 1704 olgu saptandı. En yaygın travma özelliği motorlu araç kazası (MAK) (%62,5) ile düşme (%23,8) idi. Trafik kazası mağdurları çoğunlukta yayalardı (%41,6). Kafatası kırığı-nın ortalama 27,2±17,7 düzeyinde bir yaşla birlikte erkeklerde (%78,6) görülmesi daha sıktı. MAK'lerde, kafa tabanı kırığı hastaların %51,2'sinde ve kafa tavanı kırığı da hastaların %48,8'inde gözlendi. Kafa tabanı kırığı ve kafa tavanı kırığı arasında, cinsiyet dağılımı bakımından anlamlı bir farklılık vardı (p=0,002). Kafa tabanı kırığı olgularının %67,4'üne karşı kafa tavanı olgularının %58,1'inde, MAK, travma mekanizması idi (p<0,001). SONUÇ

Intentional Traumatic Brain Injury in Ontario, CanadaDegree of Doctor of Philosophy, 2011 Graduate Department of Rehabilitation Sciences University of Toronto were used. ------------------------------------------------------The first epidemiological study on intentional TBI identified 1,409 (8.0%) intentionalTBIs and 16,211 (92.0%) unintentional TBIs. Of the intentional TBIs, 389 (27.6%) were self-inflicted TBI (Si-TBI) and 1,020 (72.4%) were other-inflicted TBI (Oi-TBI). The most common causes of Si-TBI were "jumping from high places" and "firearms". Major causes of Oi-TBI were 'fight and brawl" and "struck by objects". Si-TBI was associated with younger age, female gender, and having a history of alcohol/drug abuse. Oi-TBI was also associated with younger age and having an alcohol/drug abuse history and also with male gender. The second study on discharge against medical advice found that 446 (2.84%) TBI patients left hospitals without medical advice. DAMA was significantly associated with intentional injuries in those with self-inflicted TBI and other-inflicted ii TBI. DAMA was also associated with younger age and a history of alcohol/drug abuse.Using univariate analyses, the third study found that people with intentional TBI had significantly lower FIM gains in the motor area and significantly lower relative function gains (as measured by Montebello Rehabilitation Factor Score) in the cognitive area.Multivariate analyses of the same data showed that intentional TBI was also associated with lower cognitive relative gains, while controlling for age, gender, alcohol/abuse history, and other demographic and clinical variables. Persons with intentional TBI were found to be less likely to be discharged home, controlling for other relevant confounders.In conclusion, a person who has been injured due to assault or suicidal attempt may need more individualized care as they may be at greater risk for adverse rehabilitation outcomes. These findings regarding people with intentional TBI provide a basis for enhancing efforts on prevention of violence-related TBI and DAMA, and also for improving rehabilitation programs and discharge plans for this vulnerable population.iii
