Video-Based Communication Assessment of Physician Error Disclosure Skills by Crowdsourced Laypeople and Patient Advocates Who Experienced Medical Harm: Reliability Assessment With Generalizability Theory

Abstract: Background Residents may benefit from simulated practice with personalized feedback to prepare for high-stakes disclosure conversations with patients after harmful errors and to meet American Council on Graduate Medical Education mandates. Ideally, feedback would come from patients who have experienced communication after medical harm, but medical researchers and leaders have found it difficult to reach this community, which has made this approach impractical at scale. The Video-Based Communication… Show more

“…The VCA app was described previously [ 16 , 23 ], and this project used the same software for presenting vignettes, recording residents’ responses, and delivering feedback. For each specialty, 2 cases portrayed an angry patient response, and another 2 cases portrayed a sad emotional response.…”

“…Scores were created by averaging multiple responses to 6 Likert scale questions that were designed to assess general communication skills; thus, the continuous scores presented in this paper were derived from ordinal approximations of continuous variables [ 29 , 30 ]. The reliability and generalizability of the representative cases were analyzed and reported separately [ 23 ]. We used a 2×3 split-plot ANOVA to test the effects of time (pre-post) and specialty on communication ratings.…”

“…They are often used to assess patient perceptions when using traditional patients is unethical or impractical due to high costs and lack of time [ 32 – 34 ]. Analog patients have been shown to produce reliable ratings of physician communication skills using the VCA [ 35 ].…”

“…For VCA cases presenting harmful medical errors, panels of crowdsourced laypeople provided ratings that were consistent with those of patients with personal experience with harmful error. 22 In a single-site pre-post pilot study involving paid resident volunteers from 3 specialties, standalone VCA practice without a didactic curriculum was associated with an increase in ratings of residents' error disclosure skills. 23 Because the effectiveness of the VCA has not been assessed, we sought to test the effect of formative feedback delivered by VCA with a large multisite cohort as part of an error disclosure curriculum.…”
