Video-assisted transaortic left ventricular thrombectomy and coronary artery bypass grafting

Abstract: A 59-year-old man with cardiac dysfunction was admitted to our hospital because of thrombus formation in the left ventricle 10 days following acute myocardial infarction. Echocardiography revealed evidence of two mobile thrombi, each measuring about 2 cm in diameter. Urgent coronary artery bypass grafting and video-assisted transaortic thrombectomy were performed without making a left ventricular incision to preserve his cardiac function. Endoscopy was useful for visualizing the anatomical structures in the le… Show more

“…Endoscopy has also been used for LV thrombectomy in adults by Kikuchi et al [2] and Duarte et al [5]. Notably, Duarte et al [5] performed LV apical thrombectomy by using a 10-mm scope to access the LV in transverse aortotomy, which is a similar method to that used in the present case.…”

“…For its evacuation, an incision has to be made in the free wall of the left ventricle, and the surgeon must check the structure within the other ventricles before removal. However, this process may cause fatal cardiac dysfunction or arrhythmias [2]. Further, for patients with acute myocardial infarction, the incision and suture of the fragile myocardium may cause serious bleeding complications [2].…”

“…The most common type of endoscopic device used was rigid endoscope [1, 5, 8, 1214, 19, 22, 23, 32, 33, 36, 37], reported in 13 studies. This was followed by semi-rigid endoscopes in four [7, 9, 21, 29], flexible endoscopes in three [2, 3, 30] and mixture of rigid and flexible endoscopes in one study [11].…”
