Video-assisted minimally invasive surgery for lone atrial fibrillation: A clinical report of 81 cases

Abstract: This minimally invasive technique proves to be safe and less traumatic and presents optimistic early outcomes for patients with paroxysmal and persistent atrial fibrillation. It might find wider application if more ablation lesions could be enrolled for long-standing persistent atrial fibrillation.

“…In a study involving patients who underwent video-assisted pulmonary vein isolation using a bipolar radiofrequency ablation system, 72.5% of the patients were found to have recovered normal sinus rhythm postoperatively [10]. However, in the cited study, only 40% of the patients with long-standing AF had a normal sinus rhythm at discharge.…”

“…The development of a minimally invasive, thoracoscopic SA technique renewed interest in SA with encouraging data in non-paroxysmal AF. 5,6 To date there has been no direct comparison between the outcomes of thoracoscopic SA versus CA in de-novo LSPAF patients. We hypothesised that thoracoscopic SA would confer superior outcomes compared with CA in LSPAF.…”

“…The available data, though limited, about LAMF after stand‐alone STAF suggest that preserved LAMF in the majority of cases is due to the reversal of electromechanical remodeling associated with maintained sinus rhythm . In a study of 104 patients undergoing STAF, LAMF was preserved in 99% of patients with persistent sinus rhythm in the form of an “A” wave in echocardiography at the end of follow‐up (17 months) …”

“…The success rate may be lower with CMP III compared to CMPIV procedures, but there are no randomized trials comparing the 2 operations. Different surgical techniques with less extensive lesion sets have evolved into minimally invasive approaches that can be performed off pump through smaller incisions or thoracoscopically in selected cases without the need for concomitant open heart surgery (stand‐alone minimally invasive STAF, mini‐maze) …”
