Vibrant Soundbridge Surgery in Patients With Severe External Otitis

Abstract: The transcanal approach for the implantation of the VSB has led to postoperative complications different from those reported after the transmastoidal posterior tympanotomy approach. External otitis should be considered as a contraindication for VSB surgery by the transcanal approach.

“…If we observe Figures 1B and C, we may notice that the gains at 1 year after first fitting are slightly higher than those obtained at the last follow-up, but no significant difference was found except at 0.5 Hz. This slight decrease has been previously described also after implantation by the transmastoidal posterior tympanotomy approach (22). On the other hand, the aided effective exceeds the BC threshold by approximately 7 dB HL at 2 and 4 kHz.…”

“…However, this rate may be higher, especially when considering implantation in open cavities. Zwartenkot et al (22) described five cases (38%) of conductor wire extrusions in a series of 13 patients with severe external otitis undergoing a transcanal approach for VSB implantation. The wire extrusion was noticed at a mean postoperative period of 25 months (range, 4Y41 mo).…”

“…In short, a bony canal or groove was drilled in the external ear canal to avoid a mastoidectomy by providing an access for the electronic wire of the VSB device. The groove was covered with bone dust (17). …”

“…Three revisions were indicated because of wire extrusions and skin defects in de external ear canal, as a result of the specific surgical method of implantation, the transcanal approach in patients with a therapy resistant external otitis. The evaluation of this method has been described in detail previously (17). Middle ear inspection was indicated for eight VSB implants because of a presumed decrease in fixation of the FMT onto the incus.…”

“…In other reports, revision surgeries were performed and hearing was restored [ 41 ]. Still others have reported that despite wire extrusion in the external auditory canal, the patients are still actively using their VSB [ 42 ].…”
