VGKC-Antikörper-assoziierte limbische Enzephalitis

Abstract: Since the initial description of limbic encephalitis (LE) in 1960/1968, several subforms of this clinico-neuropathological syndrome have been identified. The best known is paraneoplastic LE. However, non-paraneoplastic forms have been reported, too. Very recently, autoantibodies against voltage-gated potassium channels have been described in association with LE. The diagnostic workup of such a case and the apparently typical good response to long-term immunotherapy of this LE subform are described.

“…In the present interesting case report, the 42‐year‐old VGKC‐NPLE patient is described as developing temporal lobe seizures and – after several months – a hippocampal lesion that is indistinguishable from hippocampal sclerosis. Similar observations have been made in two other cases with similar features (Fauser et al 2005, Soeder et al 2005). Histopathological studies of VGKCNPLE hippocampi have not yet been performed and it remains a matter of speculation if they indeed show the same pattern of neuronal loss and gliosis as “classical” hippocampal sclerosis.…”

“…LE was histologically proven in two patients and definite due to "characteristic" onconeural or voltage-gated potassium channel antibodies (VGKC-Abs) in another three patients [6][7][8]. In another five patients (#10-#14) who initially had bilateral (n = 3) or unilateral (n = 2) hippocampal swelling changing to bilateral (n = 3) or unilateral (n = 2) atrophy during the course of 3 to 22 months, LE was considered possible although these patients had no characteristic antibodies.…”
