Vertebral and Pulmonary Actinomycosis Mimicking Metastatic Lung Cancer

Abstract: Thoracic actinomycosis with spinal involvement is a rare disease. Therefore, diagnosis may be difficult. Surgical intervention, correct diagnosis, and specific long-term antibiotic treatment resulted in favorable outcome.

“…The lesions of bone metastases from LC are usually osteolytic with poor prognosis, and the prognosis of metastatic spinal cord compression of LC is even worse [2,9]. The imaging tests of metastatic LC include masses in the lobe with enlarged hilar or mediastinal lymph nodes combined with a collapsed vertebral body with spinal cord compression [10]. Here, we report a rare example of spinal cord compression resulting from soft tissue epidural MM of the thoracic spine.…”

“…Actinomycosis, also known as "the great masquerade," can present as systemic manifestations that mimic various types of malignancies in different parts of the body. For example, vertebral and pulmonary actinomycosis can resemble metastatic lung cancer, while ovarian, rectal, pelvic, and pancreatic actinomycosis may be mistaken for malignancies in their respective organs [12][13][14][15][16][17]. In the case of primary gastric actinomycosis, patients may experience symptoms suggestive of gastric outlet obstruction, leading to misdiagnosis as gastric cancer or lymphoma initially.…”

“… 40 , 41 Bony metastatic disease leads to vertebral collapse, causing spinal cord compression, which is a typical example of involvement of the central nervous system. 42 Invasion of the brachial plexus by thoracic tumours is a common example of involvement of the peripheral nervous system. 43 In addition, remote effects of cancer on the immune system activate inflammatory cascade, which would also exacerbate cancer metabolism and the injury of nerves further.…”
