Vernixuria: Another Sign of Uterine Rupture

Abstract: Uterine rupture complicates approximately 1% of trials of labor after cesarean. Classic signs and symptoms include loss of station, cessation of labor, vaginal bleeding, fetal distress, and abdominal pain. Other signs are also possible. We report a case of uterine rupture at VBAC trial that includes an unusual clinical sign of uterine rupture: vernix caseosa observed in the urine of the parturient. During labor, a bladder catheter was inserted to evaluate oliguria. Vernix caseosa and blood were found in the tu… Show more

“…The management of uterine and bladder rupture usually requires laparotomy because of fetal distress or an arrest of labor. [11][12][13][14] In VBAC patients, rupture results from traumatic separation of the dense adhesions between the uterus and bladder during labor. Thus, expectant management might result in poor healing, which in turn could lead to the formation of vesicouterine fistulas.…”

“…Gross hematuria is the most common sign of uterine rupture associated with bladder rupture. Other rare presentations such as vernixuria, meconium stained urine, urinary incontinence induced by vesicouterine fistulas, fever, and urinary tract infections have also been noted [5][6][7][8] . The diagnosis requires a high degree of suspicion and awareness.…”

“…Frequently, the constellation of signs and symptoms is difficult to distinguish those of uterine rupture from the more commonly encountered differential diagnoses such as chorioamnionitis, placental abruption or normal labor. Vernixuria [10] has been described as a unique sign of uterine rupture. Maternal signs of hypotension and tachycardia frequently occur late in the process and lack sensitivity.…”
