Ventricular Septal Rupture and Mitral Regurgitation Caused by Trauma

Abstract

We present a young patient with traumatic septal rupture following traffic accident in whom transthoracic echocardiography allowed comprehensive noninvasive assessment of location and size of the septal defect and structure atrioventricular valve apparatus.

“…Entre los años 1970 y 2005 se habían publicado 58 casos en la literatura inglesa, con una mortalidad general del 19%, pero con amplias variaciones según el tamaño de la rotura, la necesidad de cirugía correctora urgente y las lesiones traumáticas concomitantes [1][2][3][4][5] . Parmley et al 6 encontraron rotura del septo interventricular en el 5,5% de las autopsias de fallecidos con trauma torá-cico cerrado, que frecuentemente se relacionaban con otras lesiones cardíacas: contusión o rotura miocárdi-ca, traumatismo pericárdico o valvular, o infarto miocárdico [7][8][9][10] . La cirugía cardíaca urgente por RSV muestra una elevada mortalidad incluso en casos con elevada sospecha diagnóstica y rápida intervención quirúrgica, por el contrario, la cirugía electiva en pacientes estables tiene un pronóstico comparable con la cirugía cardíaca programada, aunque no disponemos de estudios comparativos 11,12 .…”
