Ventricular hypertrophy and left atrial dilatation persist and are associated with reduced survival after valve replacement for aortic stenosis

Abstract: Severe left ventricular hypertrophy with left atrial dilatation can develop from severe aortic stenosis, even without symptoms. These changes can persist, are associated with decreased long-term survival even after successful aortic valve replacement, and could be indications for early aortic valve replacement if supported by findings from an appropriate prospective study.

“…This technique is similar to logistic regression in that it allows for the detection and characterization of nonlinear trends for haemodynamic indices. All available datapoints for each patient are utilized, and rather than analysing the data in aggregate, it is adjusted for individual patients [12,13]. The non-linear models can then be used for multivariable regression analysis, in a manner similar to the time to reintervention analysis.…”

“…As postoperative LVMR occurs within one year after AVR and left ventricular mass tends to become stable thereafter [4], echocardiographic data was collected preoperatively and 12 months after surgery. Pressure gradient across the aortic valve was calculated using a simplified Bernoulli equation.…”

“…It occurs early after surgery and continues up to 10 years [2]. However, persistent residual left ventricular hypertrophy is not uncommon despite successful AVR [4,5]. Suboptimal LVMR is known to be associated with late cardiac events and poor long-term outcomes [4,5].…”

“…Patients with degenerative AS referred for transcatheter aortic valve implantation (TAVI) typically have advanced cardiac [2] and vascular [3] adverse remodeling that may not be completely reversible after valve replacement [2,4,5]. In addition, non-cardiovascular comorbidities are common in those patients [2].…”
