Ventricular fibrillation in acute myocardial infarction in Spanish patients: Results of the ARIAM database*

Abstract: Despite the considerable improvement in the treatment of AMI in recent years, the onset of either primary or secondary VF is associated with a poor prognosis. It is usually accompanied by extensive necrosis.

“…We found that despite their high-risk features, patients with pre-hospital cardiac arrest had lower rates of use of aspirin, clopidogrel, enoxaparin, β-blockers, ACE inhibitors, angiotensin receptor blockers, statin, and nitrates within the first 24 h of presentation. This finding is in agreement with similar research using different patient cohorts [10,22]. …”

“…We observed that male gender, current smoker status, tachycardia, ST-segment deviation, and higher Killip class were independently associated with pre-hospital cardiac arrest presentation in the setting of ACS. Smoking plays a role in the formation of acute thrombosis and is also associated with elevation of systemic inflammatory markers such as C-reactive protein, white blood cell count and fibrinogen [5,18,22]. These mechanisms may predispose smokers to cardiac arrest by inducing a greater degree of systemic inflammation and myocardial necrosis.…”

“…27 Our rates of these life-threatening arrhythmias are also better than rates in contemporary cohorts: 4.3%–10.2% for VT/ventricular fibrillation combined, 12,14,20 2.6%–6% for VT 2123 and 3.5–5.4% for ventricular fibrillation. 23,24 Our better rates may be attributed to patients having smaller infarctions and fewer ensuing complications from electrical and pump malfunction.…”

“…The size of ischemic area is not the absolute potential risk factors associated with ventricular arrhythmia. Other factors as young age, female gender, absence of diabetes history and inferior infarct location had been reported to be potential risk factors for primary ventricular fibrillation in acute MI [3, 4]. The leading factors of lethal arrhythmia in acute MI are inconsistent in different studies.…”
