Ventilatory Function and Intra-abdominal Pressure in Patients Who Underwent Abdominoplasty with Plication of the External Oblique Aponeurosis

Abstract: Spirometric parameters change on the 2nd and 7th postoperative days and tend to normalize on the 15th day. Postoperative pain, BMI, and increased IAP are not related to these changes. The use of compressive garments increases the IAP.

“…The ideal ventilation strategy has not yet been established, but it is expected to optimize gas exchange and pulmonary mechanics and to reduce the risk of respiratory complications. [8] Although patients undergoing abdominoplasty have healthy lungs, the pathophysiological changes induced by obesity make these patients prone to perioperative complications, such as hypoxemia, hypercapnia, and atelectasis. [9] Immediately after the induction of general anesthesia, atelectasis develops, leading to a reduction in both ventilation–perfusion ratio and pulmonary compliance.…”

“…However, there are still postoperative complications that are divided into major and minor ones. The major ones are those that need hospitalization for treatment and the minor ones are those that can be followed in the outpatient clinic (5,11,12) . Among the complications is dehiscence.…”

“…In addition to the increase of intraabdominal pressure caused by different maneuvers used to correct myoaponeurotic laxity, [9][10][11] there is also the effect of abdominal flap traction, which can be transferred to the abdominal cavity. [13][14][15] In 1996, Matarasso brought to light the hypothesis that the use of compression garments could be one of the reasons for the increase of intraabdominal pressure, thus generating a higher incidence of deep venous thrombosis and pulmonary embolism after abdominoplasty. Some authors have measured the intraabdominal pressure by using an intravesical catheter and have stated that it does increase during the early postoperative period.…”
