Venous Stasis Retinopathy in Symptomatic Carotid Artery Occlusion

Abstract: Background and Purpose-Chronic ocular ischemia is a rare form of ischemia of the eye in patients with carotid artery occlusion (CAO). The early and often asymptomatic stage of chronic ocular ischemia is referred to as venous stasis retinopathy (VSR). The aim of this study was to gain insight into the prevalence, cause, and outcome of VSR in patients with symptomatic CAO. Methods-In 110 patients with symptomatic CAO, we prospectively investigated the frequency of VSR, the association between the presence of VSR… Show more

“…38 Tentative links have been made in previous studies between the occurrence of increased arterial diameter fluctuations and the presence of vascular disturbance in both smokers and vasospastic subjects 19,39,40 and between increased retinal venous diameters and the presence of retinal ischemia and hypoxia. 41,42 On this basis, it could be hypothesized that the increased BDF identified in our IGT patients may represent an indicator that common alterations in retinal vascular function may exist in these individuals, perhaps related to an increased variation in vascular tone or compliance. Indeed, it is important to emphasize here that in our IGT sample there was a correlation between signs of retinal vascular dysfunction and plasma lipid levels, possibly suggesting that dyslipidemia itself could have a direct influence on the retinal vascular response as assessed by the DVA.…”

“…ocular neovascularisation, glaucoma, cataracts, diabetic retinopathy and macular degeneration. 11) In patients who were symptomatic with ocular events, the presence of significantly worse atherosclerotic disease was noted in the ipsilateral internal carotid artery, external carotid artery, aortic arch branch disease and vertebral artery. 12) These findings illustrate the fact that patients with carotid occlusive disease can present with a myriad of preoperative ocular symptoms which may progress and worsen post CEA.…”

“…Carotid occlusions are often asymptomatic and detected incidentally on the wrong side during investigation of TIA or stroke [8]. Sometimes chronic ocular ischaemia presenting as a 'venous stasis retinopathy' [9] and recurrent syncopal episodes [10] have also been reported. Surgical revascularisation was proposed as a possibility for preventing cognitive impairment as far back as 1950 [11].…”
