Objectives: To conduct a systematic review of facial nerve hemangiomas (FNH), focusing on patient characteristics, management options, and treatment outcomes. Design: A systematic review was performed following PRISMA guidelines, utilizing PubMed, EMBASE, Scopus, and Cochrane databases from inception to December 31st, 2022. Setting: Literature sourced from various databases providing information on FNH cases. Participants: A total of 176 patients with FNH were included in the study, identified from 40 articles meeting inclusion criteria. Main Outcome Measures: Patient demographics, lesion characteristics, pre-operative symptoms, surgical approaches, and post-operative outcomes, including House-Brackmann (HB) Grades. Results: Among the 1682 initially identified articles, 40 were included in the final review. From these, a total of 176 patients (mean age=42.7±12.8, 51.1% male) were included for analysis. Bivariate analysis showed that patients with longer pre-operative symptom duration and facial nerve sacrifice had significantly greater post-operative HB Grades (p<.001). Additionally, a strong positive correlation was observed between pre- and post-operative HB Grades (Spearman’s rho=.649). Multivariable linear regression analysis showed that both facial nerve sacrifice (β=0.86, 95%CI:0.38-1.34;p<.001) and greater pre-operative HB Grades (β=0.36,95% CI:0.20-0.53;p<.001) were associated with significantly greater post-operative HB Grades, but pre-operative symptom duration did not persist as a significant predictor of post-operative HB Grades. Conclusions: Facial nerve hemangiomas are rare lesions of the skull base affecting the temporal bone. Our findings highlight the role of pre-operative facial nerve function and intraoperative preservation of the facial nerve in predicting post-operative outcomes. Timely resection of lesions that prioritizes facial nerve preservation is critical to achieving optimal patient outcomes.

Carotid pseudoaneurysms of petrous localization are rare. They are mostly due to trauma, tumoral or infectious diseases, or a result of iatrogenic complications after skull base surgery. Symptoms such as facial paralysis are exceptional and have rarely been described in the literature until now. We report the case of a 64-year-old woman, who developed left peripheral facial paralysis induced by two carotid pseudoaneurysms in their intrapetrous section. The treatment is endovascular, despite the high morbidity rate. She was first put on antiplatelet medications, before the left carotid aneurysm was bypassed thanks to a self-expanding pipeline-type stent with flow diversion. The left peripheral facial paralysis was due to the compression exerted by the left carotid aneurysm, probably a congenital malformation. The progressive palsy recovery was fist due to the aneurysmal thrombosis, then to the secondary fibrosis.

Objectives To characterize facial nerve (FN) schwannomas (FNSs) and FN hemangiomas (FNHs) and their clinical features and management strategies, and to describe the results of cable nerve grafting after FN sectioning during tumor removal. Methods This retrospective study included 84 FNS cases and 42 FNH cases managed between July 1989 and July 2020 at a quaternary referral center for skull base pathology. Clinical details, locations, management, and results of cable nerve grafting at 1 year and during an average period of 3.12 years were evaluated. Sural nerve interpositioning was performed for patients who experienced FN paralysis for less than 1 year and underwent nerve sectioning during tumor removal. Results FNSs more often involved multiple segments compared with FNHs. The cerebellopontine angle and the mastoid segments were involved in 16 (19.1%) and 34 (40.5%) FNS cases, respectively; however, the cerebellopontine angle and the mastoid segments were involved in 0 and 7 (16.7%) FNH cases, respectively. Sectioned nerves of 99 patients (78.6%) were restored using interposition cable grafting. At the last follow-up evaluation, 56.3% of FNSs and 60.7% of FNHs attained House–Brackmann (HB) grade III. Lower preoperative HB grades were associated with poorer postoperative outcomes. For FNSs, the mean HB grades were 4.13 at 1 year postoperatively and 3.75 at the last follow-up evaluation (p = 0.001); however, for FNHs, the mean HB grades were 4.04 postoperatively and 3.75 at the last follow-up evaluation. Therefore, extradural coaptation yielded better outcomes. Conclusion FNSs can occur along any part of the FN along its course, and FNHs are concentrated around the area of geniculate ganglion. The results of cable inter positioning grafts are better in patients with preoperative FN-HB-III or less when compared with higher grades. The outcome of the interpositioning continues to improve even after 1 year in extradural coaptation.
