Transepithelial photorefractive keratectomy (Trans-PRK), laser-assisted in situ keratomileusis (LASIK), and small incision lenticule extraction (SMILE) are three mainstay refractive surgeries worldwide. The applicability, efficacy, safety, and predictability of these different techniques are quite similar. Trans-PRK has the strongest biostability, earliest return to normal corneal sensitivity but the longest recovery time, most uncomfortable postoperative experience, and possibility of corneal haze. LASIK possesses the fastest visual rehabilitation but the slowest corneal nerve reinnervation, and flap displacement is possibly lifelong. SMILE incurs no flap-related complications and has intermediate vision recovery time and biomechanics compared with Trans-PRK and LASIK. However, it lacks the cyclotorsion-compensation system, eye-tracking system, and customized treatment profile for high astigmatism or irregular corneal surface. This review aims to introduce the mechanisms, pros, and cons of these three types of refractive surgery. With full understanding, practitioners could advise patients on the most suitable treatment of choice.

Dioptric power and refractive behaviour: a review of methods and applications

Myopia is a global healthcare concern and effective analyses of dioptric power are important in evaluating potential treatments involving surgery, orthokeratology, drugs such as low-dose (0.05%) atropine and gene therapy. This paper considers issues of concern when analysing refractive state such as data normality, transformations, outliers and anisometropia. A brief review of methods for analysing and representing dioptric power is included but the emphasis is on the optimal approach to understanding refractive state (and its variation) in addressing pertinent clinical and research questions.Although there have been significant improvements in the analysis of refractive state, areas for critical consideration remain and the use of power matrices as opposed to power vectors is one such area. Another is effective identification of outliers in refractive data. The type of multivariate distribution present with samples of dioptric power is often not considered. Similarly, transformations of samples (of dioptric power) towards normality and the effects of such transformations are not thoroughly explored. These areas (outliers, normality and transformations) need further investigation for greater efficacy and proper inferences regarding refractive error. Although power vectors are better known, power matrices are accentuated herein due to potential advantages for statistical analyses of dioptric power such as greater simplicity, completeness, and improved facility for quantitative and graphical representation of refractive state.

Purpose: Trans- photorefractive keratectomy (tPRK) technique using the Amaris laser is an alternative and modified method for PRK in correction of mild to moderate myopia. We assessed and compare the clinical sequels of the two techniques. Methods: This randomized clinical trial was conducted on 60 consecutive suffering moderate grades of myopia patients referred to Vanak Ophthalmology Clinic in 2020. The patients were randomly assigned to operate as both eyes of one patient by tPRK method (using Wong baker scale) and both eyes of another patient were by PRK method. Results: The change in patients' vision (UCVA, BCVA) in one and three months of follow-up in both groups was significant compared to preoperative values, but the rate of vision improvement was significantly better than before surgery in the tPRK group. There was no statistically significant difference between the two groups in terms of average refraction score, except for the sphere difference in one month follow-up. The mean Spherical Equivalent change in the PRK group in the one-month follow-up was 1.68 ± 1.00 and in the opposite group was 4.12 ± 0.92, indicating a better improvement in the tPRK group. There was less haziness in patients in the tPRK group than those in the PRK group. The patients undergoing tPRK experienced significantly less pain than another group. There is no difference between the two groups after surgery regarding intraocular pressure. Conclusion: The results of our study showed the superiority of the tPRK method over the PRK in correcting mild to moderate myopia
