The livedoid vasculopathy is a rare condition characterized by the presence of recurrent painful ulcers in distal extremities of lower limbs. Histologically there is thickness of dermal vessels, occlusion of its light by fibrin thrombi associated with minimal inflammatory infiltrate. It might occur as an isolated condition or be associated with an underlying systemic disease, including coagulation and collagen disorders, or neoplasms. Because it is a rare disease there is no consensus for its treatment. We report a case of a 41-year-old man with painful ulcers in the lower extremities. We did not find any associated diseases. The lesions improved dramatically after treatment with anticoagulant and hyperbaric therapy. We concluded that anticoagulation associated with hyperbaric oxygenation may be benefit for the treatment of patients with livedoid vasculopathy. However, further studies should be done with a larger population to confirm our results.

Livedoid vasculopathy is a skin disease that occludes the blood vessels of the dermis. It has a pauciinflammatory or non-inflammatory nature. It is characterized by the presence of macular or papular, erythematous-purpuric lesions affecting the legs, especially the ankles and feet, and producing intensely painful ulcerations, which cause white atrophic scars called "atrophie blanche". This review includes studies and case reports found in the medical literature regarding the etiopathogenic associations of the disease, particularly those related to thrombophilia, their histopathological findings and the therapeutic approaches used in the difficult clinical management of these cases. Keywords: Leg ulcer; Livedo reticularis; Thrombophilia; Thrombosis; Venous thrombosis Resumo: A vasculopatia livedoide é uma afecção cutânea oclusiva dos vasos sanguíneos da derme, de caráter pauci-inflamatório ou não-inflamatório. Caracteriza-se pela presença de lesões maculosas ou papulosas, eritêmato-purpúricas, nas pernas, especialmente nos tornozelos e pés, as quais produzem ulcerações intensamente dolorosas, que originam cicatrizes atróficas esbranquiçadas, denominadas "atrofia branca". Nesta revisão, abordamos os estudos e relatos de caso da literatura médica referentes às associações etiopatogênicas da doença, particularmente as que se referem aos estados de trombofilia, seus achados histopatológicos e abordagens terapêuticas empregadas na difícil condução clínica destes casos.

EditorThe diagnosis and treatment of livedoid vasculopathy is a challenge to the clinician. 1 A 47-year-old patient presented with livid reticular discoloration and ulceration on both legs, aggravated by exposure to cold since the age of 26 years. Clinical examination revealed painful focal purpuric ulcerating lesions, atrophic scarring, teleangiectasia, hemosiderin deposits and hyperpigmentation (Fig. 1a). Laboratory parameters such as coagulation factors, phospholipid antibodies, ANA, ENA, ANCA, C3, C4, immunocomplexes, C-reactive protein, cryoglobulins, cold agglutinins, erythrocyte sedimentation rate, serum protein electrophoresis, and urine were normal or negative. Repeated MRIs of the brain, X-rays of the chest and ultrasound investigations of the abdomen were normal. A skin biopsy specimen revealed dermal thickened vessel walls and fibrin deposits with occasional obliteration. Since 1983
