Liver Dysfunction in Patients with Systemic Lupus Erythematosus

Objective We aimed to define the clinical features of liver dysfunction in patients with systemic lupus erythematosus (SLE). Methods The frequency and causes of liver dysfunction were examined in 206 patients with SLE. Results Liver dysfunction was evident in 123 (59.7%) of the 206 patients. Liver dysfunction in patients with SLE can be drug-induced (30.9%) or caused by SLE itself (28.5%), fatty liver (17.9%), autoimmune hepatitis (AIH) (4.9%), primary biliary cirrhosis (2.4%), cholangitis (1.6%), alcohol (1.6%) or viral hepatitis (0.8%), and it tends to be mild except when caused by AIH. Values for aminotransferase were significantly increased when AIH was the cause, whereas alkaline phosphatase (ALP) and γ-glutamyl transpeptidase (γ-GTP) were significantly increased when AIH or drugs were the cause. The liver was already dysfunctional at the time of SLE onset in 56 (45.5%) of 123 patients with liver dysfunction. Neurological involvement was more common among patients with than without liver dysfunction, whereas SLE activity and prognosis did not significantly differ between the two groups. Conclusion Liver dysfunction in the presence of SLE can be caused by many factors, but when extant at the time of SLE onset, either SLE itself or drugs can be the cause. Autoimmune hepatitis should be considered when liver dysfunction is relatively severe.

The selectins and their ligands mediate leukocyte rolling on endothelial cells, the initial step in the emigration cascade leading to leukocyte infiltration of tissue. These adhesion molecules have been shown to be key promoters of acute leukocyte emigration events; however, their roles in the development of long-term inflammatory responses, including those that occur during chronic inflammatory diseases such as systemic lupus erythematosus, are unclear. To assess participation of P-selectin in such disorders, we studied the progression of systemic lupus erythematosus-like disease in P-selectin-deficient and control MRL/MpJ-Faslpr (Faslpr) mice. Surprisingly, we found that P-selectin deficiency resulted in significantly earlier mortality, characterized by a more rapid development of glomerulonephritis and dermatitis. Expression of CCL2 (MCP-1) was increased in the kidneys of P-selectin mutant mice and in supernatants of LPS-stimulated primary renal endothelial cell cultures from these mice. A closely similar phenotype, including elevated renal expression of CCL2, was also observed in Faslpr mice deficient in the major P-selectin ligand, P-selectin glycoprotein ligand-1. These results indicate that P-selectin and P-selectin glycoprotein ligand-1 are not required for leukocyte infiltration and the development of autoimmune disease in Faslpr mice, but rather expression of these adhesion molecules is important for modulating the progression of glomerulonephritis, possibly through down-regulation of endothelial CCL2 expression.

ÖZETAmaç: Lenfositik vaskülit bir hastalık adı değil, histopatolojik bir paterndir. Dermiste damar duvarlarını infiltre eden, "manşon şeklinde" saran ve damar trasesi boyunca yerleşim gösteren lenfositler bulunmaktadır. Damar duvarlarında kalınlaşma ve endotel hücrelerinde belirginleşme ile eritrosit ekstravazasyonları da izlenir. Lenfositik vaskülit, kollajen vasküler hastalıkları da içeren birçok hastalıkta görülür. Lupus eritematozus etiyolojisi bilinmeyen, kronik bir hastalıktır, kutanöz lezyonları histopatolojik olarak temelde interfaz vakuolar dermatitidir. Dermal musinozis, perivasküler, periadneksiyel ve interstisyel lenfohistiyositik infiltrat yanısıra "lenfositik vaskülit" paterni dikkat çekici olabilmektedir.Çalışmamızdaki amaç lupus eritematozuslu hastalardaki lenfositik vaskülit paterni sıklığını ortaya koymak ve bu paternin lupus eritematozusun tanısında yararlı ek bir bulgu olup olmadığını araştırmaktır. Gereç ve Yöntem:Işık ve direkt immunfloresan mikroskobik inceleme bulguları ile lupus eritematozus tanısı alan 36 olgu çalışmanın gerecidir. Olguların klinik bilgileri hasta dosyalarından elde edilmiştir. Lenfositik vaskülit paternini tanımak için kullanılan parametreler; dermal damar duvarlarını infiltre eden lenfositler, perivasküler alanda lenfositik infiltrasyon, damar duvarlarında kalınlaşma, endotel hücrelerinde belirginleşme ve ekstravaze eritrositlerdir. Bu parametrelere göre lenfositik vaskülit paterni saptanan olgular kaydedilmiş ve yüzde oranları hesaplanmıştır. Bulgular:Olguların 23'ü (% 63.8) kadın, 13'ü (% 36.1) erkektir. Yaş aralığı 28-82'dir. 29 (%80.5) olguda süperfisyel ve derin perivasküler dermatit paterni, 25'inde (%88.8) interfaz dermatit paterni, 6'sında (%16.6) dermal müsinozis, 6'sında (%16.6) lenfositik vaskülit paterni saptanmıştır. Olguların kesin tanıları ışık ve direkt immunfloresan mikroskobik inceleme bulguları ile birlikte konulmuştur.Sonuç: Çalışmamızda literatür ile uyumlu olarak, lupus eritematozus olgularında artmış lenfositik vaskülit paterni izlenmiştir. Lenfositik vaskülit paterni ve bazı morfolojik bulgular, klinik ayırıcı tanılar içerisinde olmasa bile bizi lupus eritematozus yönünden uyarmakta ve tanıya yönlendirmektedir. There are lymphocytes settling along the vessel trace, infiltrating and surrounding the walls of the dermal vessels forming a cuff in the dermis. In case of lymphocytic vasculitis, thickening of vessel walls, swelling of endothelial cells and erythrocyte extravasations are also observed. Lymphocytic vasculitis is seen in many diseases including collagen vascular diseases.Lupus erythematosus is a chronic disease of unknown etiology and histopathologically; its cutaneous lesions are mainly interphase vacuolar dermatitis.
