Objective. To analyse the vestibular function characteristics of patients with Meniere's disease and acute hypophonic sensorineural hearing loss in order to find more reliable and objective ancillary tests that will reduce misdiagnosis and missed diagnoses. Methods. From January 2021 to December 2021, 60 healthy adults who underwent physical examination in our hospital were included in the control group, 60 patients with Meniere's disease were included in Study Group A, and 60 patients with acute low-tone sensorineural hearing loss were recruited in Study Group B. All participants underwent the caloric test (CT), video-head impulse test (vHIT), headshaking test (HST), and vestibular-evoked myogenic potential (VEMP) testing, which includes ocular vestibular-evoked myogenic potential (oVEMP) and cervical vestibular-evoked myogenic potential (cVEMP). Results. Statistical analyses of unilateral weakness and directional preponderance (DP) in the two groups of patients found no significant differences between the two groups ( P > 0.05 ). There was no statistically significant difference in the abnormal rate of vHIT and HST results between the two study groups ( P > 0.05 ). There was no significant difference in the wave latencies, interwave intervals, and amplitudes of cVEMP and oVEMP, among the three groups ( P > 0.05 ). Conclusion. This study found that factors affecting CT, vHIT, HST, and VEMP results included age, head posture and position during testing, stimulus type, manipulation method, and control of muscle tone, and also those that are related to the testing instrument, statistical software, and manipulation procedures, resulting in different excitation rates and testing parameters. The small sample size prevented a comprehensive assessment of the differences in vestibular function between patients with Meniere's disease and acute hypotonic sensorineural hearing loss, and a larger sample size will be investigated in the future to provide useful insight into the diagnosis, treatment and differentiation of Meniere’s disease, and acute hypotonic sensorineural hearing loss.

Болезнь Меньера (БМ) – многофакторное заболевание внутреннего уха, характеризующееся головокружением, шумом в ушах и нейросенсорной потерей слуха. Заболевание не имеет определенной этиологической причины. Развитие БМ зависит от целого ряда факторов, которые способны приводить к эндолимфатической водянке. Основным морфологическим признаком БМ является эндолимфатический гидропс – увеличение объема эндолимфы, заполняющей перепончатый лабиринт внутреннего уха, и растяжение Рейснеровой мембраны. Различают 3 основных варианта течения заболевания. Рост количества случаев БМ у лиц молодого возраста, а также отсутствие скрининга у лиц с повышенным риском (люди в возрастной категории от 20 до 50 лет со сниженным слухом на 1 или оба уха, с отягощенным семейным анамнезом по данному заболеванию, с травмами головы и/или уха, аутоиммунными заболеваниями) делает актуальной проблему разработки методов ранней диагностики и комплексного лечения БМ в Республике Беларусь. Meniere’s disease (MD) is a multifactorial disease of the inner ear characterized by dizziness, tinnitus and sensorineural hearing loss. The disease has no definite etiological cause. The development of MD depends on a number of factors that can lead to the development of endolymphatic dropsy. The main morphological feature of MD is endolymphatic hydrops: an increase in the volume of the endolymph filling the membranous labyrinth of the inner ear, and stretching of the Reissner’s membrane. There are three main variants of the disease course. The increasing number of MD cases in young people, as well as the lack of screening in high-risk populations (individuals aged 20 to 50 years with impaired hearing in one or both ears, having family history aggravated with this disease, with head and/ or ear injuries, or with autoimmune diseases) make the issue of developing methods for early diagnosis and comprehensive treatment of MD in the Republic of Belarus relevant.

Choroba Ménière’a, zwana także wodniakiem błędnika bądź wodniakiem endolimfatycznym, to choroba przewlekła charakteryzująca się tetradą objawów – zawrotami głowy, szumami usznymi, postępującym upośledzeniem słuchu i uczuciem pełności w dotkniętym uchu. Występuje z częstością 50–200 na 100 000 dorosłych, z większym odsetkiem u kobiet. Choć choroba została opisana i powiązana z uszkodzeniem ucha wewnętrznego już w 1861 r. przez francuskiego lekarza Prospera Ménière’a, to jej etiologia wciąż nie jest do końca poznana. Znane są przypadki rodzinnego występowania choroby, jednak u większości osób zaburzenie to pojawia się w sposób sporadyczny. Przypuszcza się, że na wystąpienie patologii mogą składać się czynniki genetyczne i środowiskowe. Według licznych badań naukowych do potencjalnych przyczyn można zaliczyć: mutacje genetyczne, zaburzenia autoimmunologiczne, zaburzenia hormonalne i choroby wirusowe. Dotychczas nie osiągnięto międzynarodowego konsensusu w sprawie postępowania diagnostycznego i terapeutycznego w chorobie Ménière’a, co jest źródłem kontrowersji. W diagnostyce i prognostyce choroby Ménière’a dużą rolę odgrywa audiometria tonalna, jednak mogą być także stosowane takie badania jak: wideonystagmografia, test fotela obrotowego, test przedsionkowych miogennych potencjałów wywołanych, posturografia, vHIT, elektrokochleografia, a coraz większe znaczenie mają badania obrazowe rezonansem magnetycznym, w tym MRI 3D FLAIR. Istnieje kilka różnych algorytmów postępowania w leczeniu choroby Ménière’a. W ostrych atakach choroby Ménière’a zastosować można leki przeciw zawrotom głowy (leki przeciwhistaminowe, benzodiazepiny, leki przeciwcholinergiczne) i środki przeciwwymiotne (metoklopramid i domperidon). W przewlekłej terapii i prewencyjnie według rekomendacji ważne jest zastosowanie odpowiedniej diety uwzględniającej większą podaż wody oraz ograniczenie spożywania soli, kofeiny i alkoholu, a także zastosowanie rehabilitacji przedsionkowej i słuchowej. W farmakoterapii wiele schematów uwzględnia także stosowanie batahistyny, diuretyków oraz dobębenkowych iniekcji sterydów i gentamycyny. W przypadku braku powodzenia leczenia farmakologicznego jako ostatni rzut można rozważyć leczenie chirurgiczne – labiryntektomię i neurektomię przedsionkową. Niektóre zalecenia uwzględniają też terapię PPT, jednak wymaga ona dalszych badań w celu potwierdzenia swojej skuteczności. W pracy przedstawiono patogenezę, diagnostykę i niechirurgiczne możliwości leczenia choroby Ménière’a, a także kontrowersje związane z tymi obszarami.
