Vascular access use and outcomes: an international perspective from the dialysis outcomes and practice patterns study

Abstract: Background. A well-functioning vascular access (VA) is essential to efficient dialysis therapy. Guidelines have been implemented improving care, yet access use varies widely across countries and VA complications remain a problem. This study took advantage of the unique opportunity to utilize data from the Dialysis Outcomes and Practice Patterns Study (DOPPS) to examine international trends in VA use and trends in patient characteristics and practices associated with VA use from 1996 to 2007. DOPPS is a prospec… Show more

“…These differences in patient groups may impact the different incidence of CTS in Japan and other countries. In addition, the ratio of vascular catheter use, associated with a high incidence of inflammation, was much lower in Japan (2%) than in the USA (25%) and in European countries (10% in Germany, 12% in Italy, and 21% in Spain) [40]. This may also be associated with the difference in incidence of CTS between Japan and other countries.…”

“…Increasingly higher numbers of patients start dialysis with catheters, and the waiting time to obtain arteriovenous fistulas (AVFs) is progressively increasing [18,19]. Nevertheless, AVFs are widely accepted as the preferred hemodialysis vascular access type.…”

“…Two previously conducted studies reported respectively a 7-fold and an 8-fold increase in BSI risk for CVC compared to AVF [16,20]. Catheter care and use have changed significantly since the first publication of KDOQI vascular access guidelines in 1997 (updated in 2006) [26,40] that recommended that >40% should be dialyzed with an AVF and <10% with a CVC [41]. However, despite efforts to reduce it, CVC use has increased in Canada and in the US [2,40].…”
