Neurologic complications and therapy in herpes virus in· fectionsAn overview on the diversity of neurologie eomplications of infections with different human herpes virus strains is given. Encephalitis, meningoencephalitis and the Guillain·Barr~· syndrome are of major importance. Affection of single cra· nial nerves and mononeuropathies occur in a lesser frequency, while myelitis and isolated disturbances of the autonomie nervous System are rare complications. Therapeutically the application of acyclovir in herpes simplex and varicella zoster virus infections has given encouraging outlooks, whereas no convincing results exist with respect to cytomegalo virus and Epstein-Barr virus infections. Zusammenfassung In der vorliegenden Arbeit ist die Vielfalt neurologischer Komplikationen bei Erkrankungen mit den verschiedenen Herpesvirusarten dargestellt. Von Bedeutung sind Enzephalitiden und Meningoenzephalitiden; ferner kommen Radikulopolyneuritiden, Ausfälle einzelner Hirnnerven und Mono· neuropathien vor. Myelitiden sowie isolierte Ausfälle des aut6nomen Nervensystems gehören dagegen zu den selteneren Kompl ikationen. Therapeutisch ist der Einsatz von Acyclovir bei Erkrankungen mit Herpes simplex und Varizellazoster-Virus erfolgversprechend, während für Zytomegalie-und Epstein.BarrVirusinfek tionen bisher keine aussagekräftigen Untersuchungsergebnisse vorliegen. Die Herpesviren gehören zur Gruppe der DNA-Yiren. Sie werden in verschiedene Gruppen unterteilt: die Herpessimplex-Gruppe mit den zwei antigenetisch differenten Typen Typ I (Herpes labialis) und Typ 11 (Herpes genitalis), das Herpes-B-Yirus, das Yarizella-Zoster-Yirus, das EpsteinBarr-Yirus sowie das Zytomegalievirus. Die Herpesviren verursachen sehr unterschiedliche Krank.-heitsbilder, die sich jedoch alle auf Gewebe ektodermalen Ursprungs beschränken. Im ersten Teil dieser Arbeit werden zunächst die den einzelnen Yirusuntergruppen zuzuordnenden Krankheitsbilder des Nervensystems beschrieben. Im zweiten Teil wird über deneitige Möglichkeiten der Behandlung von Herpesvirusinfektionen und ihre Erfolgsaussichten berichtet.
Herpes-simplex-VirenInfektionen mit den beiden antigenetisch verschiedenen Herpes·simplex-Yirus (HSV)-Gruppen, Typ I (Herpes la· bialis) und Typ 11 (Herpes genitalis) verlaufen in 99 %der Fälle klinisch inapparent (78). Die gefürchtetste Komplikation einer HSY-Infektion ist die Herpesenzephalitis. Sie wird deshalb in ihrer Pathogenese und Symptomatologie ausftihrlicher dargestellt. Die häufigste Ursache der nicht-epidemischen oder sporadischen akuten Yirusenzephalitis in den USA ist das HSY Typ I (96): 13-19 %aller Enzephalitiden im Erwachsenenalter werden durch diesen Yirustyp hervorgerufen (48, 96). Pathogenetisch scheint es sich in 30 %der Fälle um eine primäre Infektion zu handeln (96). Tierexperimentell und durch Autopsiestudien wurde nachgewiesen, daß ein intraaxonaler Transport des Virus vom Nasopharynx über die Area cribriforrnis entlang dem N. olfactorius in den Temporallappen erfolgen kann (1, 65).
