Background: There is no study about the evaluation of incidence rates of tonsillectomy, adenoidectomy, and ear surgeries before and during the coronavirus disease 2019 (COVID-19) pandemic and the impact of pandemic peaks on the incidence rates of these operations in the world, particularly in Iran. Objectives: This study aimed to evaluate the impact of the COVID-19 pandemic on the number of most common otolaryngologic operations in the Iranian pediatric population. Methods: This retrospective study was performed in a referral hospital. The data of target patients over the period of March 21, 2018, to November 22, 2021, were collected. The number of surgeries at the period of any peak plus before and during the COVID-19 pandemic was compared. Results: A total of 6 290 patients from the Health Information System of Amir A’lam hospital, Tehran, Iran, were enrolled. In this study, 52.9% and 46.5% of the patients were male and female, respectively. There were 3 main types of surgeries, such as adenoidectomy, tonsillectomy, and ear surgeries, with mean ± standard deviation (SD) age of 10.20 ± 9.4, 11.45 ± 10.8, and 44.19 ± 14.6 years, respectively. There was a significant difference in terms of the total number of operations between before (n = 5070) and during the COVID-19 pandemic (n = 1220) (P < 0.001). The amount of this difference was much greater in tonsillectomy (n = 3736 vs. n = 546) and adenoidectomy (n = 134 vs. n = 21) than in ear surgeries (n = 1200 vs. n = 653). Conclusions: The COVID-19 pandemic significantly reduced the number of most common otolaryngologic surgeries in the Iranian pediatric population. The reason is that the restrictions during the pandemic might have a potential effect on reducing the transmission of tonsil-adenoid gland infection.

The emergency presented through the COVID-19 pandemic exposed the need to adopt remote, technology-driven solutions and make healthcare services more resilient. To do so, we need technological applications (i.e., telemedicine) that are designed and tailored to the end-users (i.e., chronic patients) needs and the type of healthcare service they get (i.e., cancer care). The requirements above are especially relevant to Greece, being a country with numerous sparsely populated regions (e.g., islands, regions at the borders) and a deteriorating access to healthcare for all citizens. Trying to address such diverse problems and needs, there have been multiple, different telemedicine and telecare projects in Greece in the past years. To support the future design and implementation of such endeavours, in this study we translated a questionnaire measuring the acceptance of telemedicine by patients and adapted it to the Greek context. We continued by running a small-scale pilot with 73 Greek women with breast cancer to assess the adapted instrument for its reliability and construct validity. The created questionnaire had good overall and internal reliability scores for most sub-scales. Factor analysis did not identify the same number of latent dimensions as the original theoretical model. Reverse wording items needing to be recoded were identified, and items that could be omitted in future versions of the questionnaire. Increasing the sample size for the purposes of a longitudinal study, the construct, convergent, and discriminant validity are elements to be further examined in future studies. It is envisaged that the creation of this questionnaire will support the adoption of telemedicine by Greek healthcare services into more routine areas of patient care provision.

ZusammenfassungDie Ereignisse bezüglich einer Reform der Notfallversorgung überschlagen sich, und ein klares Konzept ist noch nicht erkennbar, wenngleich überfällig. Kliniken und Praxen erleben im Fachgebiet der Hals-Nasen-Ohrenheilkunde (HNO) einen deutlichen Zuwachs entsprechender Patienten, die Gründe sind mannigfaltig: von dem Bedürfnis Betroffener einer sofortigen Abklärung bestimmter Symptome bis hin zu veränderten Notdienststrukturen. Verschärft hat sich die Situation durch das jüngste Urteil zur Sozialversicherungspflicht der Poolärzte mit einer „Notbremse“ im ärztlichen Bereitschaftsdienst bspw. der Kassenärztlichen Vereinigung (KV) Baden-Württemberg. Zu allem Überfluss wurde die vom Gemeinsamen Bundesausschuss erstellte Erstfassung der Richtlinie zur Ersteinschätzung des Versorgungsbedarfs in der Notfallversorgung (Ersteinschätzungsrichtlinie) durch das Bundesministerium für Gesundheit beanstandet und liegt aktuell auf Eis. Es ist allen Akteuren klar, dass in jedem Fall eine grundlegende Reform zur Filterung und Steuerung der Patientenströme notwendig wird. Die Vorschläge reichen von einer Aufklärung der Bürger zur Erlangung einer basalen Gesundheitskompetenz über die Wiedereinführung der Notfallgebühr bis hin zu den wahrscheinlichen Szenarien einer zentralen Notfallnummer, eines gemeinsamen Tresens (i.e. vertragsärztliche Notfallpraxis und entsprechende Klinik-Notfallambulanzen in unmittelbarer räumlicher Nähe) und einer Triage durch geschultes Personal mit Vergabeoptionen in die geeigneten Versorgungsbereiche. Die Regierungskommission für eine moderne und bedarfsgerechte Krankenhausversorgung hat mit Stellungnahmen zur Reform der Notfall- und Akutversorgung zu den Bereichen „Rettungsdienst und Finanzierung“ und „Integrierte Notfallzentren und Integrierte Leitstellen“ Empfehlungen abgegeben. Hierzu sollen a.e. im Verlauf des Jahres, nach Möglichkeit unter Beteiligung der Fachgesellschaften und Berufsverbände, belastbare Grundlagen geschaffen sein, auch verknüpft mit der Hoffnung, die dringend notwendige Entlastung von medizinischem Personal in Klinik und Praxis einzuleiten.Das vorliegende Manuskript beschreibt die gesundheitspolitische Historie sowie aktuelle Probleme der Notfallversorgung, wobei Lösungsvorschläge mit Bezug auf die HNO-ärztlichen Besonderheiten in den Fokus genommen werden. Das vorliegende Positionspapier ist verbunden mit einem Appell an Selbstverwaltung und Politik, nunmehr zeitnah ein tragfähiges Konzept für die Notfallversorgung in Kraft zu setzen, da Finanzierung und Personalverfügbarkeit immer kritischer werden und der ungeregelten Welle an Notfallpatienten eine ordnende Hand gereicht werden muss.
