Variation and treatment of vessels in laparoscopic right hemicolectomy

Abstract: The core area of laparoscopic radical extended right hemicolectomy includes the pancreatic neck, duodenum, and right gastroepiploic vessels. The difficulty lies with the standard treatment of the GCT. A medial-to-lateral approach is more in line with the principle of no-touch in tumor surgery and is applied from lower to upper, inside to outside, and left to right, for both the vessels and plane of dissection. Familiarity with vascular variation and the management of vessels in key areas are essential for succ… Show more

“…Application of the principles of CME in RH is technically challenging, especially when minimally invasive approaches (laparoscopy and robotic surgery) are used [1,7]. This is principally due to the highly variable arterial supply to the right colon [8,9]. A proper preoperative understanding of the vascular anatomy of the right colon is paramount to ensure adequate oncological resection with minimal intra-operative complications [8,9].…”

“…Because of the lack of specific manifestations of colon cancer in the early stage, most patients are already in the middle and advanced stages at the time of diagnosis. Recurrence and metastasis are the main factors affecting the efficacy [ 3 ]. Looking for new treatment methods and strategies to improve the treatment effect of colon cancer has become a hot spot in clinical research.…”

“…As the surgeons' understanding of mesenteric vascular variation deepened and D3 dissection was actively attempted in the daily surgical practice field, it was found that various anomalies of the feeding artery and draining vein were quite common in the right colon region. The ileocolic artery is present in almost all patients; in 40%, it passes anteriorly to the SMV and travels to the right and in 60%, it branches from the SMA and proceeds posteriorly to the SMV [41,[52][53][54].…”
