Variants of Preexcitation—Specialized Atriofascicular Pathways, Nodofascicular Pathways, and Fasciculoventricuiar Pathways:

Abstract: The majority of patients with variants of the preexcitation syndrome present with specialized atriofascicular pathways that seem to originate from remnants of the specialized AV ring tissue. Nodofascicular and fasciculoventricular pathways exist and may give rise to preexcitation, although their functional role in participation of clinical arrhythmias still needs to be elucidated. In the present study, both a fasciculoventricular pathway and a nodofascicular pathway acted as a bystander.

“…Catheter ablation is accomplished by identifying the proximal Mahaim Accessory Pathways and distal insertions and, ideally, the recording of a proximal pathway potential at the tricuspid annulus or a distal one on the right ventricular free wall (see Figure 6). 5,[13][14][15][16][17] Pathway potential recording may be facilitated during atrial pacing. Since most of the Mahaims are mapped on the lateral tricuspid annulus or right free wall underneath the valve, the use of supportive long sheaths that stabilise the ablating catheter may be very helpful.…”

“…None of the authors mention if junctional automaticity occurred during current delivery at the right midseptal region. Others, like Kottkamp [38], did not need to ablate the Mahaim fiber, which had a bystander role, and ablation of the slow AV nodal pathway was all that was needed to make the patient asymptomatic. …”

“…The fasciculoventricular pathway is a rare variant of preexcitation taking off from the bundle of His or the bundle branches and inserting into the ventricular septum [35,36]. Since Gallagher et al [37] published the largest series of fasciculoventricular pathways (six patients) in 1981, few additional cases were reported [38][39][40][41][42]. A number of reasons account for the paucity of publications, such as its rarity and underdiagnosis either because no cardiac arrhythmias occur or because of the small amount of ventricular preexcitation on the electrocardiogram (Figure 24.28).…”
