Variants in clopidogrel-relevant genes and early neurological deterioration in ischemic stroke patients receiving clopidogrel

Abstract: Background: Early neurological deterioration (END) is common in acute ischemic stroke (IS). However, the underlying mechanisms for END are unclear. The aim of this study was to evaluate the associations of 16 variants in clopidogrel-relevant genes and interactions among these variants with END in acute IS patients receiving clopidogrel treatment. Methods: We consecutively enrolled 375 acute IS patients between June 2014 and January 2015. Platelet aggregation was measured on admission and after the 7-10 days of… Show more

“…Furthermore, the synergistic effect of genes interactions may contribute not only to platelet activity but also to stroke prognosis. Yi et al [ 37 ] showed that carriers of high-risk interactive genotypes not only exhibit a significantly lower platetet inhibition, but are also at a higher risk of early neurological deterioration than noncarriers (hazard ratio (HR) = 2.82; p = 0.003). The molecular mechanisms responsible for the above characteristics still remain unclear.…”

“… 38 The rs6809699 SNP and rs16863323 SNP in the LD block that also contains the rs3732759 SNP of the P2RY12 gene have been reported to be associated with intravenous immunoglobulin resistance in Kawasaki disease patients 39 and early neurological deterioration in acute ischemic stroke. 40 However, no previous studies have shown that the rs3732759 SNP of the P2RY12 gene and SNPs in the same LD block are associated with pain. Individuals who are homozygous for the major allele of the rs3732759 SNP may have higher P2RY12 expression, which may be more active in microglia and predispose these individuals to neuropathic pain, including PTP.…”

“…The current methods for identifying high-risk individuals depend largely on the assessment of the clinical and neuroimaging features present at the time of onset. In recent years, researchers have focused more on END in ischemic stroke (8)(9)(10). It was reported that the maximum body temperature and fever burden within 24 h after endovascular thrombectomy were independent predictors of END in ischemic stroke patients with large vessel occlusion (11).…”
