Variant Adrenal Venous Anatomy in 546 Laparoscopic Adrenalectomies

Abstract: Importance: Knowing the types and frequency of adrenal vein variants would help surgeons identify and control the adrenal vein during laparoscopic adrenalectomy. Conclusions and Relevance: Understanding variant adrenal venous anatomy is important to avoid bleeding during laparoscopic adrenalectomy, particularly in patients with large tumors or pheochromocytomas. Surgeons should anticipate a higher probability of adrenal vein variants when operating on pheochromocytomas and larger adrenal tumors.

“…Display of the proper catheter position using a C-arm CT is a current additional option. Perfect positioning should permit selective sampling of the entire adrenal gland; this is difficult, however [37]. It is particularly important to distinguish the right adrenal vein leading directly into the inferior vena cava from small accessory hepatic veins ( • " Fig.…”

“…Nevertheless, previous anatomical reports gave little attention to the orientation and orificial location of the right adrenal vein1314, which is of paramount importance to AVS procedures. Recently, the presence of Dyna CT and its ability to delineate the vascular structures have increased the success rate of AVS1015.…”

“…The inferior phrenic vein could be preserved in most instances. On the other hand, anatomic variants of the right adrenal vein exist in about 13% of cases and have been found to be related to certain patient-and tumor-related features (12). Anatomic variations include: the absence The superior pole of the kidney is seen below the energy device, and the caudal aspect of the adrenal gland and its surrounding fat have been completely dissected free from it.…”
