Variables influencing manometric parameters of deglutitive and non‐deglutitive upper esophageal sphincter: A study of 89 asymptomatic participants

Abstract: Background High‐resolution manometry overcomes limitations imposed by axial and radial asymmetry as well as swallow and respiration‐induced movement of the upper esophageal sphincter (UES), allowing for its reliable use in evaluation of dysphagic and reflux patients. The aim of this study was to determine normative values and their variability across position, sex, age, height, weight, BMI, and volume for clinically relevant deglutitive and non‐deglutitive UES parameters. Methods We studied 89 asymptomatic vol… Show more

“…The P‐HRM‐I literature also suggests that UES IRP and/or hypopharyngeal IBP are reliable predictors of post‐chemoradiotherapy stricture 24 and are improved when the CP muscle is disrupted by dilatation 33 and cricopharyngeal peroral endoscopic myotomy (c‐POEM) 12 . Here, we confirmed previous reports of age‐related impairment of UES function 21,34–39 . The patients exhibiting a range of evidence for a UES disorder were found to demonstrate the greatest aspiration scores.…”

“…12 Here, we confirmed previous reports of age-related impairment of UES function. 21,[34][35][36][37][38][39] The patients exhibiting a range of evidence for a UES disorder were found to demonstrate the greatest aspiration scores. This is consistent with a report demonstrating association between aspiration and evidence for impaired UES relaxation.…”

“…16 Manomeric assessment of the UES is limited due to variability in normative data and confounding effect of UES reactivity. 12,[24][25][26] In our study, HRM UES pressures did not correlate with FLIP UES-DI in our sample. FLIP is performed during sedated upper endoscopy when UES reactivity is diminished.…”

“…5,11 However, manometric UES pressures are highly variable even in asymptomatic individuals, which is likely due to reactivity and the hyperdynamic behavior of the UES. 12 At the present moment, a key gap in our knowledge of esophageal pathophysiology is with respect to the mechanism of generation of supra-esophageal symptoms. As many as 1 in 5 patients reports symptoms such as cough, hoarseness, and globus.…”
