Introducción: Evali por sus siglas en inglés (e-cigarette or vaping associated lung injury), describe la lesión pulmonar asociada al uso de cigarrillos electrónicos o vaper. En 2019 los Centros de Control y Prevención de Enfermedades (CDC) de Estados Unidos, reportaron más de 2.807 casos de pacientes con daño pulmonar. Objetivo: Realizar un análisis clínico, diagnóstico y terapéutico del uso de los cigarrillos electrónicos o vaper. Métodos: Se realizó una revisión documental descriptiva, a través de la búsqueda en base de datos: PubMed, Medline, Scielo, Elsevier y Google Académico, en los últimos 5 años, en idiomas inglés y español. Desarrollo: El Evali, es más frecuente en adolescentes y adultos jóvenes, entre 18 y 35 años. Los cigarrillos electrónicos poseen combinados con tetrahidrocanabinol (THC) y mezclas de nicotina, los cuales son inhalados algunas veces como terapéutica para erradicar el tabaco y otras hasta por moda. Los síntomas predominantes son: tos no productiva, dolor torácico pleurítico, dificultad para respirar, hipoxemia e insuficiencia respiratoria aguda; además algunos casos con diarrea, vómitos y otros síntomas generales. Los criterios de casos confirmados son: -Uso de cigarrillo electrónico ("vapear") durante 90 días antes del inicio de síntomas. -Radiografía y/o tomografía axial computarizada de tórax (TAC) con imágenes de opacidad o en vidrio esmerilado. -Ausencia de infección pulmonar. Conclusiones: El impacto de afecciones respiratorias graves y muertes, deja preocupación e inquietud mundial. El daño del cigarrillo electrónico o vaper en las vías respiratoria y pulmones debe seguir siendo objeto de estudio. The predominant symptoms are: non-productive cough, pleuritic chest pain, shortness of breath, hypoxemia and acute respiratory failure; also some cases with diarrhea, vomiting and other general symptoms. The criteria for confirmed cases are: -Use of electronic cigarettes ("vaping") for 90 days before the onset of symptoms. -X-ray and/or computed tomography of the thorax (CT) with opacity or ground glass images. -Absence of lung infection. Conclusions: The impact of serious respiratory illnesses and deaths leaves concern and concern worldwide. The damage of the electronic cigarette or vaper in the respiratory tract and lungs must continue to be studied. Electronic cigarette, or vaping, product use-associated lung injury (EVALI) is an increasingly recognized entity with the potential for severe pulmonary toxicity. We present the case of a young man first evaluated at a tertiary care center in the United States in 2019 with newly diagnosed testicular cancer with acute respiratory failure, which was initially attributed to possible metastatic disease but eventually determined to be related to EVALI. This case highlights the clinical features of EVALI, the potential diagnostic dilemma that can arise with EVALI when occurring in the setting of malignancy and the importance of inquiring about vaping use among patients with malignancy, especially in adolescents and young adults.
