Value of Free-Run Electromyographic Monitoring of Lower Cranial Nerves in Endoscopic Endonasal Approach to Skull Base Surgeries

Abstract: Objective? The main objective of this study was to evaluate the value of free-run electromyography (f-EMG) monitoring of cranial nerves (CNs) VII, IX, X, XI, and XII in skull base surgeries performed using endoscopic endonasal approach (EEA) to reduce iatrogenic CN deficits. Design? We retrospectively identified 73 patients out of 990 patients who had EEA in our institution who had at least one CN monitored. In each CN group, we classified patients who had significant (SG) f-EMG activity as group I and those wh… Show more

“…SSEPs 16 and TcMEPs 15 monitor the dorsal column and the motor pathways in the spinal cord and reduce neurological deficits after spinal surgeries. EMG has been used for placement of pedicle screws 33 , as well the incidence of cranial nerve deficits in during thyroidectomy 34 and skull base tumor removal 35 . The techniques though useful, have not be validated based on the alarms and subsequent surgical interventions used to modify the procedure.…”

“…This study was approved by the local institutional review board for retrospective review of the clinical outcomes. The subsequent description of the neurophysiological monitoring techniques has been discussed in detail in earlier publications, [35][36][37][38][39] and a brief overview is given below.…”

“…Significant free-run EMG activity, when present for prolonged periods of time (≥ 100 msec) from a CN, was reported to the surgeons as 1 alert and was also recorded in the patient's records. 38,39 Triggered EMG responses were obtained upon request using a constant-current stimulator. All negative and positive responses were communicated to the surgeons.…”
