Value of cytopathologist‐performed ultrasound‐guided fine‐needle aspiration as a screening test for ultrasound‐guided core‐needle biopsy in nonpalpable breast masses

Abstract: Fine-needle aspiration (FNA) of breast masses in the United States has been on the decline for the last decade and has been largely replaced by ultrasound-guided core-needle biopsy (UG-CNB). Some studies show core-needle biopsy (CNB) is superior to FNA in terms of absolute sensitivity, specificity, and inadequate rate. However, the importance of a skilled aspirator, experienced cytopathologist, and immediate cytological evaluation (ICE) in FNA is often not considered. CNB is more expensive, invasive, risky, an… Show more

“…The use of core biopsy (CB) is being increasingly advertised but its procedure is more cumbersome, expensive and time consuming as compared to FNA procedure. [5][6][7] "Although fine-needle aspiration (FNA) biopsy of the breast has been shown to be a safe and accurate technique, many surgeons question whether it is reliable enough to replace excisional biopsy. If FNA biopsy is followed by an excisional biopsy for confirmation, it would seem that the cost of diagnostic workup would be increased, but it has been seen that FNA biopsy is cost effective even when followed by an excisional or frozen section biopsy for confirmation.…”

“…Although core biopsy is preferred to FNAC in most developed countries, its procedure is more expensive and time consuming as compared to FNAC [8][9][10] . Th is study was conducted to evaluate the sensitivity and specifi city of FNAC in the diagnosis of breast tumors in our institution by correlating it with histopathology fi ndings.…”

“…Rare cytopathologists are further extending their practice to the performance of core biopsies 132 and placement of radiologic clips when clinically indicated. 134 In one office visit, 87% of patients with breast masses were able to be diagnosed with only ultrasound-guided FNA; core biopsies were performed to enable diagnosis of invasion for patients with positive FNAs and to resolve nondiagnostic FNAs. 134 Interventional cytopathologists are appropriately resurrecting breast FNA.…”
