Value and Interpretation of Resection Margin after a Colonoscopic Polypectomy for Malignant Polyps

Abstract: PurposeThis study was designed to compare the clinicopathologic findings of an endoscopic polypectomy for malignant polyps with subsequent surgery and to evaluate the appropriateness of the pathologic finding criterion of the resection margin as an indicator for surgery in cases of malignant colorectal polyps.MethodsWe examined the clinicopathologic characteristics, complications and prognoses among the patients who underwent a colonoscopic polypectomy in both our hospitals and at other hospitals from April 20… Show more

“…62.4% of patients with malignant polyps were males, similar to benign polyps’ group (60.28%); in the literature, the majority of subjects with malignant polyps were males, prevalence ranging from 51–88% [ 19 , 30 , 31 , 32 , 33 , 34 , 35 ]. The presence of three or more polyps is significantly higher in male patients ( p =0.012) and is associated with a higher risk for HGD ( p <0.0001).…”

“…In one, all be it small, study 75% of incompletely resected polyps were associated with an adverse outcome. [20] It should be noted that even when incomplete resection is reported, absence of residual disease in the surgical specimen is the rule (94% in one study) rather than the exception [21]. This is likely due to diathermy electrofulguration of the remnant.…”

“…The European recommendations state that tumour cells within 1mm of the margin represents a positive margin [22], with some authors arguing then >2mm represents the true safe margin [21].…”

“…In addition, PIT has the potential to be useful during endoscopic resections of polyps. Prior studies have demonstrated that 51.3% of patients undergoing polypectomy alone had positive resection margins [18]. Performing PIT to the resection bed after polypectomy can be advantageous to treat residual disease and decrease the rate of recurrence and need for further operation.…”
