Valproate Monotherapy in an SRI-Intolerant OCD Patient

Abstract: Sir: Risperidone, a recently introduced novel antipsychotic, is associated with a low incidence of seizures. During premarketing testing, seizures occurred in 0.3% (9/2607) of risperidone-treated patients (with dosages unrevealed), two in association with hyponatremia.1 To our knowledge, no postmarketing risperidone-associated seizure has been reported. We describe an elderly schizophrenic woman who developed a single seizure after 2 days of coadministration of risperidone, sulfamethoxazole-trimethoprim, and a… Show more

“…For instance, it was reported that some anticonvulsants such as valproic acid, carbamazepine, and phenytoin have been used for the adjuvant treatment of obsessive-compulsive disorders. On the contrary, it was also reported that zonisamide and topiramate can induce or aggravate obsessive-compulsive disorder [1,2].…”

“…In the case mentioned, the OCS returned when divalproex was stopped, and reintroduction of divalproex improved OCS. [71] There are reports of successful use of valproate in an SRI intolerant OCD[72] and clozapine-induced OCS. [73] In an eight-week, double-blind, placebo-controlled trial of divalproex sodium in autistic spectrum disorder, there was significant improvement in repetitive behaviors measured by Children's YBOCS in divalproex group and hence it may be useful to treat OCS in BD.…”

“…In OCD, the literature examining the use of anticonvulsants is limited to case reports and case series, with inconsistent results. Cases have been documented showing positive results using gabapentin [CoraLocatelli et al, 1998a] and valproate [Cora-Locatelli et al, 1998b;Delito, 1994] in treatment-resistant OCD, and there are mixed results in case series of carbamazepine [Iwata et al, 2000;Jenike and Brotman, 1984]. A trial of eight patients with OCD refractory to SRI treated with adjunctive lamotrigine, a glutamatergic anticonvulsant agent, did not show benefit.…”
